The inspection was an examination of the activities conducted under your license as they relate to radiation safety and to compliance with the Nuclear Regulatory Commission (NRC) rules and regulations and the conditions of your license. The inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspector.

3. LICENSEE CONTACT Mark Luttrell, RSO Next Inspection Date: PROGRAM SCOPE 4. TELEPHONE NUMBER (765) 412-6988 07/23/2019 This was an unannounced, routine inspection at the licensee's facility in Reynolds, Indiana. The licensee had 20 fixed gauges during the onsite inspection.

None of the gauges had been used since October 2016. The RSO was as authorized, and he reported to the Maintenance Manager. The RSO stated that the week before the onsite inspection, the licensee filed for Chapter 11 bankruptcy.

The licensee had 12 Berthold Technologies Model LB-7440-D-CR fixed gauges, and 8 Vega America Model SHLD 1 fixed gauges. Based on the licensee's recent history, the acting MIB Chief determined that the next routine inspection is due on 7 /23/19. Performance Observations The inspector: ( 1) asked the RSO to provide the written notification regarding the Chapter 11 bankruptcy as required by 10 CFR 30.34(h);

(2) observed two Model SHLD1 fixed gauges stored in a secured cabinet that was posted with a "Caution Radioactive Material" sign, and used an NRC-owned, calibrated survey instrument to conduct an independent ambient exposure rate survey of the surfaces of the gauges and the result was 0.33 milliRoentgen per hour; (3) observed that all of the gauges' shutters were locked closed; (4) observed the licensee's calibrated survey instrument; (5) noted that the RSO stated that there were no incidents, fire, flood, loss, or theft of the gauges since the last inspection; ( 6) the licensee conducted gauge inventories, closed shutter position checks, and shutter lock checks at 6 months intervals; (7) used an NRC-owned, calibrated survey instrument to conduct independent ambient exposure rate surveys of the surfaces of selected gauges that were accessible and there were no concerns, and (8) spoke to the RSO about the written notification requirement in 10 CFR 30.36(d)(3) realizing that the 24 months term ends in October 2018. No violations ofNRC regulatory requirements were identified as a result of this inspection.
