SUBJECT: FERMI INSPECTION REPORT 50-341/90009(DRP) ,

Dear Mr. Gipson:
This refers to the inspection conducted on May 16 through June 18,1999, at the Fermi 2
facility. The enclosed report presents the results of this inspection.
During this inspection period, the plant was operated in a safe and conservative manner. In
l particular, the operators' decision to trip the plant following the unexpected loss of the reactor
- recirculation pump was performed promptly and in accordance with procedures and license
requirements. Although accidental tripping of the reactor recirculation pump occurred during a
'
maintenance activity, maintenance personnel were thorough in the investigation of the root
cause of the event. Plant organizational response to fouling in several balance-of-plant heat
exchangers was prompt and effective. ;

However, the plant configuration with Emergency Diesel Generator 11 removed from service )
while the opposite division's Standby Liquid Control System B was inoperable raised a l
Technical Specification 3.8.1.1.c compliance question. Though the compliance aspects of this l
'
Technical Specification (TS) issue are still under review, the occurrence of this situation
reflected some weaknesses in operations performance. Specifically, weak procedural
guidance, communications issues, and configuration control weaknesses were evident. Of
particular concem was the poor communication between operations and other site
organizations in the initial dispositioning of the acceptability of the configuration. In addition,

when this configuration was changed by opening the Standby Liquid Control Pump B breaker ;
l for troubleshooting, the change in availability of the pump was not communicated and therefore ;
not evaluated. Though overall operations performance during this inspection report period was
I good, as reflected in the excellent response to the unexpected recirculation pump trip, the
- circumstances which led to the plant configuration in question indicate that enhancements may
l be necessary in the above mentioned operations performance areas.
