- 11. LASALLE COUNTY STATION: Unit 1 Ill. DOCKET NUMBER: 050-373 IV. EVENT DESCRIPTION While performing LOS-FP-W2, for the Diesel Fire Pump OA, fuel oil was noticed leaking from a flange on the flexible fuel line connect-Ing the day tank to the diesel.
V. PROBABLE CONSEQUENCES OF THE OCCURRENCE During the performance of this surveillance, diesel fire pump OB was available and operable.
This is the second broken flexible fuel line occurrence reported for either of the fire pump diesels.
Even though this is the second occurrence of this nature, no generic problem is evident. Future performance of the hoses will be monitored for any recurrence of this type of problem.
VI. CAUSE The event occurred as a result of a broken flexible fuel line. The fuel line is approximately 21 feet long and was supplied by Commins Engine Sales. .
Vll. CORRECTIVE ACTION The immediate corrective action to resolve the problem was to put the OA diesel fire pump out of service to stop the fuel oli leak. The broken flex 1ble fuel line was then replaced by a temporary copper tube fuel line. The diesel was then test run under LOS-FP-W2, to verify problem had been temporarily corrected.
The long term corrective action to prevent recurrence will be done under work request #L16011. The temporary copper tubing will be replaced with a new flexible hose fuel line purchased from Cummins Engine Sales. The diesel will then be test run to verify problem is corrected.
