Narrative Report for IER 50-366/1982-53 On 5-8-82, with Hatch Unit 2 in cold shutdown for the planned I refueling / maintenance outage, engineering personnel were i performing a Logic System Functional Test of the Low Pressure Coolant Injection Mode of RHR, HNP-2-3153 revision 7 "LPCI LSFT". While performing the LSFT the personnel effected temporary changes to the procedure to resolve procedural errors and inadequacies, the personnel performing the LSFT f ail ed to obtain the Unit 2 Shift Foreman's signature on the temporary change to HNP-2-3153 revision 7 until after completion of the LSFT. This oversight is in violation of HNP-9 revision 19 " Procedure Writing, Use and Control".
The involved personnel misinterpreted the applicable procedural controls of HNP-9 revision 19 and have since been obtaining temporary instructed in the approved methods of procedural changes.
