| Title | External Contamination Found on a Radiopharmaceutical Delivery Package |
| Event Description | The package was received July 15, 2008 by the VA Medical Center in Fayetteville, North Carolina.
The VHA National Health Physics Program was notified of the event on August 7, 2008, at 9:30 AM ET. A wipe test performed on the external surface of the package upon receipt on July 15, 2008 indicated a removable contamination level of 254 [disintegrations per minute per square centimeter] as compared to the regulatory limit of 220 [disintegrations per minute per square centimeter]. Surveys inside the package did not indicate elevated contamination. The package contained radiopharmaceuticals with Tc-99m and was shipped from Cardinal Health in Fayetteville, North Carolina. The vendor/shipper also serves as the delivery carrier. The shipper/delivery carrier was immediately notified of the contaminated package after the receipt survey on July 15, 2008 by a nuclear medicine technologist at the VA Medical Center. |
