Thefourthchangeinvolvedthecapability toisolatecontainment serviceairinModes1,2,3,and4.Specificallyf werequested Table3.6-1bechangedtoaddvalvePCR-40tolistAandvalvePA-342tolistEandtodeletevalvePA-243andablindflange(Item47)fromlistE.Attachment 2tothislettercontainsafigureanddrawingwhichshowthelocationoftheabovevalves.Figure9.8.3fromtheupdatedFSARshowsthephysicallocationofthevalves.Drawing1-2-PO-01 showstheisometric locationofthevalvePCR-40,whichwouldreplaceamanuallyoperatedvalveatthatlocation.

~l):1)abrasionmarksappearedonthedoorframebottomledgewheretwoofthedoor'ssheetmetalscrewsweremakingcontactasthedoorwasopened,2)largedentsinplateBwereobserveddirectlybelowtheabrasionpointstherebysuggesting thatamechanical force,impartedonplateB,mayhaveresultedinforcingplateAupwardsandalsoincreasing thesizeofthegapbetweenplatesA5B,3)theRTVsealant,previously installed inthegapbetweenplatesAEB,wasfoundmissingtherebyallowingthepossibility ofcondensation torunfromatopplateAdownintotheinsulation tofreezeandpushplateAupwardsand4)mineralresidue,indicating condensation flowacrossplateA(fromwarmsidetocoldside)anddowntowardthegapbetweenplatesA8B,wasobserved.Duringthesecondinspection, thefollowing verification weremade:1)itwasverifiedthatnoneoftheotherdoorframebottomledgesexhibited significant damage(dents);thetypeofwhichwasfoundinbay3and2)itwasverifiedthatonlybay14(inadditiontobay3)wasmissingtheRTVsealantinthegapbetweenplatesAandB.Asaresultoftheseinspections, DaycoSheettietalInc.wasorderedtoremovebothplateAandplateBfrombays3and14toallowinspections foricebuildupintheinsulation.
Aftertheplateswereremoved,itwasverifiedthattheinsulation inbay3washeavilyloadedwithice.Theinsulation in.bay14,however,revealednoevidenceofwateroricebuildup.Basedohtheobservations madeduringtheseinspections, itisapparentthatdoor3-RplateAwasforcedupwards(andincontactwiththedoor'ssheetmetalscrews)asaresultofacwbination ofmechanical impacttransmitted fromplateBandinsulation expansion asaresultoffreezingcondensation.
Duringtheinsulation, itbecameobviousthatnotonlydidtheblowstoplashBforceplateAupwardsbutalsoresultedinwideningthegapbetweenplatesA8Bwhichresultedinahighprobability ofincreasing condensation flowintotheinsulation.
PageThree~Uponccmpleting theinspections described intheattachedinvestigation report,DaycoSheettietalInc.replacedallofthefrozeninsulation alongthebay3doorframebottcmledgeandplatesAEBwerereinstalled inbothbay3andbay14.Inaddition, RTVsealantwasinstalled alongtheentirelengthofthegapbetweenplatesA8Binbothbays.TheRTVsealantisconsidered apermanent fixinprohibiting condensation flowdownthroughthegapandintotheinsulation.

Inanattempttodetectanydamagetothedoorsthatwouldresultinanincreaseinthedoor's.openingtorque,theinitialopeningforcewillbere-measured justpriortopowerescalation attheendofeveryicecondenser basketweighingproject.Thisinvestigation wasdonebyK.A.Tonerthy3,1979LER79-028EventDescription AndProbableConsequences:
Whileperforming aroutinesurveillance testitwasdiscovered thatthetorquerequiredtoopenlowericecondenser inletdoor3-Rexceededthelimitidentified underT.S.3.6.5.3.CauseDescription AndCorrective Actions:Thesheetmetalscrewslocatedalongthebottomofthedoorwerediscovered toberubbingagainstthesurfaceattheedgeofthedoorframerequiring excessive forcetoopenthedoor.Thedoorframeedgewasdepressed toallowproperclearance, andaretestwasperformed withsatisfactory results.(see supplement)
