gPer+ATTACHMENTTOAEP:NRC:00413y<o>ation1TechnicalSpecification6.8.1requiresimplementationofprocedures,.12-0HP-4021.018.004"DrainingorAdjustingLevelintheSpentFuel.Pit"detailstheprocedureforraisinglevelintheSFP.'ontrarytotheabove,thisprocedurewasnotusedforincreasinglev'elintheSFPbetweenMarch15,1980andMarch17,1980,whenthepoolwasoverfilledandcross-contaminationof'thedemineralizedwatersystemoccurred.~Resense:Duringthefuelstoragerackmodification,demineralizedwaterwasbeingusedatthespentfuelpitfortwopurposes:1)asasupplyofcleanwaterfordecontaminationoftheoldracksbeingremoved,and2)asasourceofprimingwaterforavacuumpumpusedforcleaningthespentfuelpitfloor.Thedemineralizedwatersystemcontaminationinvestigationrevealedthatthecrosscontaminationofthedemineralizedwaterheaderoccurredthrough.thehoseconnectionusedforthevacuumpumppriming:Theoneendofthehosewasfounddisconnectedfromthevacuumpumpandthefreeendlefthangingsubmergedinthespentfuelpit.Itisbelievedthattheshutoffvalveatthedemineralizedwaterheaderhoseconnectionhadbeenleftopen.Thiswouldpermitwatertoflowintothepitwheneverthedemineralizedwaterboosterpumpwasrunning.Theboosterpumpisrequiredtosupplywateratthiselevation.Whentheboosterpumpwasshutdown,thelowheaderpressurewould.permitwatertobesiphonedfromthespentfuelpitintothedemineralizedwaterheader.Thedailyspentfuelpitlevelreadingsshowagradualincreaseinleveluntilthespentfuelpitwasfoundoverflowingintothefueltransfercanal.Thelevelwasthenloweredinaccordancewithapprovedprocedures.Duringthetimeleadinguptothecrosscontaminationofthedemineralizedwaterheaderandtheoverflowingofthespentfuelpit,therewasnointentionalactiontoraisethelevelinthespentfuelpitandthereforethecitationforfailuretouseanapprovedprocedureforincreasingtheleveldoesnotseemwarranted.*Itisfeltthattherootcauseforthisentireincident,theincreaseinspentfuelpitlevel,thedilutionoftheboronconcentrationbelowtherequiredminimum,andthecrosscontaminationofthedemineralizedwaterheader,canbeattributedtoabreakdowninthecontroloftheactivitiesassociatedwith/thespentfuelrackmodification.Positivestepshavebeentaken,asindicatedbelow,topreventthere-occurrenceofasimilarevent.
