RadiationAccidentMonitoringInstrumentR-15Awasinoperableformorethansevendays.InaccordancewiththeGinnaStationOffsiteDoseCalculationManual(ODCM)ProcedureCHA-RETS-ODCM,SectionIII.A.2.a.iv.andTableIII-2,thefollowingconstitutesaSpecialReportoutliningtheactiontaken,thecauseoftheinoperabilityandtheplansandscheduleforrestoringthesystemtooperablestatus.OnFebruary25,1997,at0942EST,SPINGAirEjectorRadiationMonitorR-15Awasdeclaredinoperableforcalibration,percalibrationprocedureCPI-RAD-MON-SPING4.Thiscalibrationnormallytakesmorethanonedaytopeform.DuringcalibrationactivitiesonFebruary26,R-15AChannel9(highrangenoblegas)wasexhibitinginconsistentreadings.FurtherexaminationrevealedalooseconnectorbetweentheChannel9detectorandthejunctionbox.Tighteningtheconnectorseemedtocorrecttheproblem.Thecalibrationcontinuedoverthenexttwodays,andnootheranomalieswerenoted.OnFriday,February28,whenthecalibrationinvolvingthechecksourcesbegan,Channel9againexhibitedinconsistentreadings.ResolutionofthisproblemrequiredreplacementofthedetectorandcableforChannel9.Theseactivitiescouldbecompletedinlessthanoneday.Thus,overtimewasnotconsiderednecessary,andfurtherworkwassuspendedfortheweekend.97040i0365'st70324PDRADOCK05000244SPDR Page2OnMonday,March3,acorrectiveworkpackagewaspreparedwhichprovideddirectionstoreplacetheChannel9detectorandcable.AftercompletingthecorrectiveworkandpreparingtorestoreR-15Atoservice,I&CnoticedthatanotherR-15Achannel(Channel8,gammabackground)wasnotfunctioningasexpected.Correctionofthisproblemwasestimatedtooccurafterthe7dayclockexpired,evenwitharoundtheclockefforts.Thedecisionwasmadetodefercorrectiveworkuntilthenextday,whichwouldextendR-15Ainoperabilitybeyond7days.OnMarch4,thecorrectiveworkpackagewasmodifiedtoincludethereplacementofChannel8detector.Thisreplacementproceededmoreexpeditiouslythanexpected.Afterthisdetectorwasreplaced,calibrationwascompletedontheremainderoftheR-15Asystem.R-15Awastestedsatisfactorily.R-15Awasdocumentedasreturnedtoserviceat1049ESTonMarch4,67minutespastthe7daylimit.Verytrulyyours,RobertC.Mecryxc:Mr.GuyS.Vissing(MailStop14C7)PWRProjectDirectorateI-1Washington,D.C.'20555U.S.NuclearRegulatoryCommissionRegionI475AllendaleRoadKingofPrussia,PA19406GinnaSeniorResidentInspector 0Ih'
