| DESCRIPTION OF THE EVENT:

| With the unit at full power, monthly periodic test 2.9 revealed the high steam flow trip setpoint comparator FC-2-494 (steam generator C) was set 4.076 X 10" lb/HR high and could not be adjusted into acceptable range. This is in a non-conservative direction and constitutes a degraded mode of operation permitted by T.S.-3.7.A.

| This is reportable per T.S.6.6.2.b.(1).

| 2.

| PROBABLE CONSEQUENCES and STATUS of REDUNDANT EQUIPMENT:
| |
| Each steam line is provided with two steam flow channels. High steam flow on 1/2 channels, for 2/3 steamlines, coincident with low steam line pressure or low Tave, will initiate safety injection, to mitigate the effects of a main steen line break.
| |
| With the trip point of comparator FC-2-494 not at the correct value, one channel of high steam flow for steam generator would not have tripped at the design value. Since the redundant channel for this steam generator, as well as the channels for the other steam lines, were functional at the correct setpoints, the health and safety of the public were not affected.

| The setpoint error was caused by a malfunctioning comparator.
| |

| ==IMMEDIATE CORRECTIVE ACTION==
| |
| The High Steam Flow trip bistable, previously placed in trip to perform the periodic test, was maintained in trip.
| |
| 5.

| SUBSEQUENT CORRECTIVE ACTION:

| The flow comparator was replaced and the new comparator calibrated. The channel was returned to service.
| |
