Investication revealed that the switch was only installed on I
io 3 ; the "A" Diesel and no problems have been encountered. There have been j
[o 1il l no similar occurrences.
This is reportable per T.S.6.9.1.8(i).

Reportabic Occurrence Report No.
50-368/78-13 2.
Report Date: 03/22/79 3.
Occurrence Date: 11/07/78 4
Facility: Arkansas Nuclear One - Unit 2 Russellville, Arkansas 5.
Identification of Occurrence:
Hotification by the Emergency Diesel Generator vendor that a speed switch has a potential problem which could result in a component failure.
6.
Conditions Prior to Occurrence:
Steady-State Power Reactor Power 0
MWth Hot Standby Net Output 0
MWe Cold Shutdown Percent of Full Power 0 Refueling Shutdown Load Changes During Routine Power Operation 0
Routine Startup Operation Routine Shutdown Operation Other (specify)
X Preoperational, Start-up or Power Ascension Tests 7.
Description of Occurrence:
The speed switch in the Emergency Diesel Generator had the potential problem'which could result in a component failure.

Reportable Occurrence Report No.
50-368/78-13 8.
Designation of Apparent Cause of Occurrence:
Design Procedure hbnufacture X
Unusual Service Condition Including Environmental Installation /
Construction Component Failure (See Failure Data)
Operator Other (specify) 9.
Analysis of Occurrence:
The decision to replace the speed switch was made knowing that when the motor is at approxiuately 810 rpm, it vill excite the generator and should it nalfunction during this start-up, it could have caused overloading and possible further damage to the generator.
There was no hazard to the health and safety of the public since -the other diesel generator was unaffected and the reactor had never been critical.

Reportable Occurrence Report n'o.
50-368/78-13 10.
Corrective Actio..:
The switch was replaced with one manufactured by another vendor and was satisfactorily tested.
11.
Failure Data:
There have been no similar occurrences.
