(2) Contrary to Title 10 of the Code of Federal Regulations (CFR) 20.1101 ( c ), since 2013 the licensee failed to

periodically (at least annually) review the radiation protection program content and implementation. Specifically, the

licensee had not reviewed its radiation program since the last inspection.

The cause of the violation was that the RSO was not aware of the requirement. As corrective action, the RSO

committed to create a recurring preventive maintenance work order every 6 months to remind him to perform shutter

checks, inventories, leak tests, and audits at the required frequencies.

PROGRAM SCOPE

This was an unannounced, routine inspection of a iron casting plant located in Cadillac, Michigan. The plant produces a

variety of parts primarily for the automotive industry. The licensee possessed one specifically licensed device, a

Thermo Fisher Scientific Model 5201. The gauge is used in the melting process to measure the level of raw material in

the plant's cupola. There are two authorized users on the license: the RSO and the assistant RSO. The licensee does not

perform any installation, maintenance, repair or removal activities relating to the gauge. Instead, the manufacturer is

called in for these services.

Performance Observations

The inspector toured the casting facility and observed the gauge in its installed location. Independent confirmatory

surveys in the vicinity of the gauge matched readings from the licensee's micro-R meter. The inspector interviewed the

RSO and assistant RSO, who were both knowledgeable of radiation protection concepts and procedures. Inventory of

the gauge was demonstrated by work permits requiring lockout of the gauge whenever personnel enter the cupola every

seven to eight weeks to perform routine maintenance.

Two violations were identified during this inspection, as documented on Part 1 of this form concerning leak tests of the

gauge and audits of the radiation protective program. Specifically, the RSO forgot to perform a leak test on the fixed

gauge at the required frequency (every three years) since 2013 and had not reviewed the radiation protective program

since the last inspection in 2013.
