Licensee Event Report (LER) 95-008 is attached. This event is reportable to the NRC per 10CFR50.73(a)(2)(v)(A), as a condition that alone could have prevented the fulfillment of the safety function of a system that is needed to shutdown the reactor and maintain it in a safe shutdown condition.
