| i An unplanned radioactive liquid release contrary to ETS 2.4.1.a and reportable under ETS 5.6.2.a.

| Conditions Prior to Occurrence:
| |
| Mode 3 hot shutdown and Mode 5 cold shutdown.
| |
| Description of Occurrence:
| |
| L At 0600 on August 17, 1979 it was reported that an unplanned radioactive release of approximately 82,800 gallons occurred whe the Turbine Building sump was inadvertently pumped to the settling pond following secondary demineralizer resin sluicing operations.
| |
| The calculated activity for this release was 1.22 mpe with a total activity of 525 uci.
| |
| At 1500 on August 22, 1979 it was reported that an unplanned radioactive release of approximately 27,000 gallons occurred when the Turbine Building sump was inadvertently pumped to the settling pond. The calculated activity for this release was.76'mpc with a total activity of 71.12 uCi.

| Designation of Apparent Cause:
| |
| The cause of the inadvertent release is attributed to a lack of administrative control. Procedural controls were not available to adequately control the processing and subsequent release of the activity in the Turbine Building sump. The activity accumu-lating in the sump as a result of secondary demineralizer resin sluicing operation, originated from a primary to secondary leak.

| ==Corrective Action==
| |
| The isolation valve from the Turbine Building sump has been closed, locked.and placed under administrative control. A short-term instruction was written to establish controls for the proce 1
| |
| sing and subsequent disposition of the water in the Turbine Buil ing sump. Final ccrrective action was accomplished through the installation of a radiation monitor.(RM-L7) with a'ssociated valves and interlocks. Appropriate Chemistry Procedures and Operating Procedures have been revised to incorporate this moni-tor.
