| Description of Occurrence: On April 20, 1980 at 1430 hours, during the performance of the Service Water System Refueling Relief Valve Test, ST 5075.03, maintenance personnel observed that SW PSV 3962 was continuously discharging and would not rescat even when pressure was reduced well below its relieving point.

| This valve provides overpressurization protection for the service water discharge header.
| |
| Being in Mode 6, the station did not enter the action statement of Technical Specifi-cation 3.7.4 which requires there be two independent service water loops operable in Modes 1, 2, 3, and 4.
| |
| Designation of Apparent Cause of Occurrence:
| |
| SW PSV 3962 failed because the adjust-1l ing ring that controls the relieving pressure had backed off and was preventing the valve from seating. There was no spring tension holding the valve closed. Essentially, the valve was continuously leaking by.
| |
| The locking mechanism on the adjusting ring was not holding the ring at its desired position. The bellows was also found to be SW PSV 3963 in the other service ruptured but was not a cause of the valve failure.
| |
| water loop was checked and had a ruptured bellows, but it was not continuously leaking by and was only slightly out of tolerance in the conservative direction.

| Under Main tenance Work Order 80-1319; SW PSV 3962 was repaired

| prevent it from backing off. SW PSV 3963 also had its bellows replaced. The repaired 1l valves were successfully tested under ST 5075.03 on May 23, 1980 and placed back in service.
