, Test, ST 50>J.01, was not performed within surveillance requiremenc frequency Conditions Prior to Occurrence: The unit was in Mode 6, with Power (MRT) = 0 and Load (Gross MRE) = 0.
Description of Occurrence: During a review of the surveillance test schedule on
. April.23, 1980, the Shift Supervisor discovered that ST 5080.01, Off-Site AC Sources Lined Upand Available Weekly Test, had gone past its surveillance require-ment late date of April 23,.1980 at 0440 hours.
This is being reported to document the failure to meet a surveillance requirement of Technical Specification 3.8.1.2 which requires, in Modes 5 and 6, an off-site AC-source to be determined operable at least once per seven days by verifying correct
~ breaker' alignment and indicated power availability.

ST 5080.01 was completed at 1030 hours the same day and was late by only five hours and fifty minutes.
Des'ignation of Apparent Cause of Occurrence: The cause of this occurrence was opera-
. tor error since ST 5080.01 was overlooked on the surveillance test schedule. Given the workload and activity level created by the refueling outage, a procedural defi-ciency was evident since administrative controls were not sufficient to assist the Shif t Supervisor.by identifying pending late surveillasce tests.
Anslysis of Occurrence: There was no danger to the health and safety of the public
'cr to station personnel. Jba emergency diesel generator was available to supply safety related. equipment in the event of a total loss of off-site power.
Surveillance Test ST, 5080.01 was completed without any discrepancies.
Corrective' Action: There is no evidence that the Shift Supervisor has difficulty in maintaining the surveillance test schedule when normal shift work loads exist. There-
? fore, to preclude the consequences of an ina.ccurate schedule review during periods of
' increased work activity, administrative controls which will inform the Shift Supervi-sor of surveillance tests approaching their late date have been implemented.
