LER ATTACHMENT - RO #1-81-26 Facility: BSEP Unit No. 1 Event Date: 2-14-81 This event and the one reported in LER 2-81-09 occurred because a spring-loaded pin on the mechanical slip-on connection between the flow paddle and transmitter of the flow indicator / recorder became loose due to vibration in the effluent discharge line causing an inaccurate flow signal to be sent to the flow recorder.
In each case, the slip-on connection was reas-(
sembled with a new pin, the flow indicator / recorder and transmitter were checked for calibration and system was returned to normal operation. As both of these events occurred due to the same problem, it is believed that another transmitter of diffarent design with its accompanying mechanical connection hardware will eliminate the source of the problems encountered.
Plant preventative maintenance procedures are being revised to calibrate this indicator / recorder and transmitter on a quarterly basis. It is believed that this will help reduce the number of system failures until a permanent fix is in effect.
