At about 5:30 p.m. on Monday, No vembe r 16, 1961, a Florida Power and 1.1ght Coripany mechanic observed that both doors in the Unit 4 contatonent personnel hatch were open.                                               The unit was in refueling shutdown condition but, since fuel handling activittes were in progress at the time, this door configuration was unintenti'* 2: and deviated fran the requirements of (Cchnical Specification 3.10.1.
