CONFIDENTIAL During conversations with the staff, inquiries were made concerning Question 2 of the Licensed Operator Fitness-for-Duty Questionnaire. The questionnaire addressed a Fitness-for-Duty occurrence on November 29, 1993, involving a licensed operator.
