The Wood River Refinery reported that a shutter mechanism on a fixed gauge (Ohmart Model SH-F1B, S.N. 2508CN) was found stuck in the open position during a routine survey and maintenance check. The gauge contained a 3.7 GBq (100 mCi) Cs-137 source. Staff were unable to return it to the shielded position.
