A package containing three sources of Eu-152 (10 millicuries each) was shipped from canberra to Lockhead Martin at DOE facilities located at Idaho Falls. Upon arrival at the DOE site, it was noted that the chipping container, labeled as DOT Radioactive III, was damaged.                 Upon opening the conteiner it was found that a cap of one of the internal leaded shields containing the sources was loose and did not contain a source. DOE did not take possession of the shipment, but put it into secured storage.

Tho Fed Ex driver and truck were fcund four hours later in Idaho Falls. A su::vey of the inside of the truck f ound the missing sealed source. The source was retrieved and placed into a shielded container and placed in secured storage by DOE.

The licensee informed NRC that the sources were properly packaged in the same container in which they had received the materials from the original supplier and that there was no damage to the package at the time it left their Connecticut facility. The licensee is coordinating with Fed Ex to develop a dose assessment for this incident.
