ANO-2 is a Combustion Engineering Model 2815 designed plant that began operation in December 1978. The 2P98 inspections performed on both SGs (SGA and SGB) were a 100% full length bobbin coil examination, with the aception of the tube area below the sleeves. Also performed in 2P98 was a 100% rotating pancake coil probe (RPC) inspection in the hot leg (HL) expansion transition (ET) region. The RPC used consists of a 0.115 inch pancake coil with both axially-oriented and circumferentially-oriented coils. The RPC was also utilized for confirmation of bobbin coil calls.

Manual primary and secondary analyses were puforud by Level IIA and Imol III qualdied data analysts who were quali6ed by pro 6ciency examinations on previous ANO-2 data. Indications identified by either or both the primary and secondary manlysts wem reviewsd by a lead analyst for resolution. Final review of the resolved data was performed by an Entergy Operations review team that consisted of a Level III analyst on each shift. Part of the review team analyst's responsibihty was to review data   l that was dispositioned by the resolution analyst, u well u a sampling of tubes identified as NDD       i (no detectable degradation).

require that complete results of all ANO-2 steam generator (SG) tubing inservice inspections performed during the report period be submitted to the NRC on an annual basis. Attached is the Steam Generator Tubing Inservice Inspection Report which pisa the results from j

ANO-2's scheduled mid-cycle outage (2P98) inspection. The inspection was coaW l

during February and March 1998.

The 2P98 ina-* ions performed on both SGs were a 100% full length bobbin coil examination, with the exception of the tube area below installed sleeves. A 100% rotating pancake coil probe (RPC) inspection in the hot leg expansion transition region was also performed in each SG. 'Ihe RPC used consists of a 0.115 inch pancake coil with both axially-oriented and circumferentially-oriented coils. The RPC 'was also utilized for confirmation of bobbin coil calls. Likewise, the Plus Point coil was used to confirm small radius U-bend indications - A plugged tube summary was included in the report submitted to the NRC on March 14,1998 (2CANO39802).

Operations to submit an annual report to the NRC that outlines the details of the inspections that were performed during the reporting period. The report shall include:

Number and extent of tubes inspected.

i 2.

Location and percent of wall-thickness penetration for each indication of an imperfection.

Identification of tubes plugged or sleeved.

ANO-2 is a Combustion Engineering Model 2815 designed plant that began operation in December 1978. The plant has two recirculating steam generators (SGs), each having 8411 high temperature mill annealed Inconel Alloy 600 tubes with a 0.75" outer diameter and a 0.048" wall thickness. The tubes were full depth explosively expanded into the tubesheet. The tube supports in the lower part J

l of the SG are of an eggerate (EC) type which consists of an array ofintersecting one inch wide and two inch wide flat carbon steel plates at each support elevation. There are seven full EC support plates, two partial EC support plates, two partial drilled support plates, and five strap supports called batwings (BWs) for the horizontal run of the tubing. The BW supports consist of two diagonal and three vertical straps. Two tube sleeve types, Babcock & Wilcox (B&W) kinetic and l

Asea Brown Boveri tungsten inert gas welded, are currently installed in the SGs.

Entergy Operations performed one inservice inspection (ISI) of the SG tubing during this reporting period during a mid-cycle outage (2P98) in February and March 1998. A summary of the ISI performed is included below:

2P98 INSPECTION 4

I The 2P98 inspections performed on both SGs (SGA and SGB) were a 100% full length bobbin coil examination, with the aception of the tube area below the sleeves. Also performed in 2P98 was a 100% rotating pancake coil probe (RPC) inspection in the hot leg (HL) expansion transition (ET) region. The RPC used consists of a 0.115 inch pancake coil with both axially-oriented and circumferentially-oriented coils. The RPC was also utilized for confirmation of bobbin coil calls.

Likewise, the Plus Point coil was used to confirm small radius U-bend indications.

Table 1 list the initial inspection scope. No expansions were required.

Tubes unable to allow passage of a 0.600 probe were tested with smaller diameter probes The test frequencies were 400, 200,100 and 20 kHz. The RPC test frequencies were 300,200,100 and 20 kHz. Manual primary and secondary analyses were puforud by Level IIA and Imol III qualdied data analysts who were quali6ed by pro 6ciency examinations on previous ANO-2 data. Indications identified by either or both the primary and secondary manlysts wem reviewsd by a lead analyst for resolution. Final review of the resolved data was performed by an Entergy Operations review team that consisted of a Level III analyst on each shift. Part of the review team analyst's responsibihty was to review data that was dispositioned by the resolution analyst, u well u a sampling of tubes identified as NDD i

(no detectable degradation).

.In-situ pressure testing was perforrned on the larger indications at the eggerates (5) and in the freespan (3). The tubes tested did not leak at main steam lire break conditions nor did they burst below three times the normal operating diff' rential pressure (3 AP).

e INSPECTION RESULTS The bobbin inspections were performed from the hot and cold leg sMes of both SGs. In SGA,156 tubes were found to have ever-ad indications requiring repair, due to either exhibiting flaw-like distorted support indications (DSIs), distorted freespan indications (DFIs), volumetric indications, or circumferential indications in the ET region Tubes containing these indications are detailed in Table 2. In SGB,121 tubes were found to have confirmed indications requiring repair, due to either exluViting flaw-like DSIs, DFIs, volumetric indications, or circumferential indications in the ET region. Tubes containing these indications are detailed in Table 3. Tubes with confirmed crack-like indication by RPC were removed from service by mechanical plugging Circumferential indications were also stabilized. No sleeves were installed.

The ET region indications were primarily circumferential cracks with some axial cracking present in a few tubes.

The-eggerate indications were predominantly axial cracks based upon RPC characterization and observations from previously pulled tubes. Sludge p' e indications are generally d

. attributed to flow induced vibration across the horizontal portion of the tubing at the upper support structure. Three bobbin indications were identified in low row U-bends and were examined using the Plus Point probe for characterization. No indications were found in those 6,Wions.

It should be noted that the percent wall thickness is no lortger assigned based on bobbin results.

Initial calls are made from the bobbin results and then diagnostic testing with RPC is performed The diagnostic testing will either confirm the initial call or disposition the call as a non-crack-like indication. Those calls that were confirmed (with the exception of wear) were repaired Those calls that did not confirm were reviewed by the Entergy Review Team for final disposition.. Tables 4 and 5 list thom tubes with indications that were returned to service. These tubes contain the wear calls

< 40% TW asociated with the batwing supports.

CONCLUSIONS In summary, a comprehensive eddy current examination was performed with the analysis process also utilized for the 2R12 ' ;+h Both SGs were tested 100% full length (except below sleeves) with the bobbin coil and 100% at the hot leg ET region with RPC.

The circumferential cracks detected in 2P98 exhibited a size that was bounded by previous inspections. These cracks would be expected to exhibit burst pr:ssures well above the criteria of Regulatory Guide (RG) 1.121. Previous in-situ pressure tests were performed on flaws which bound thoseidentified in 2P98.

The eggerate axial cracks identified during 2P98 would be ~W to exhibit burst pressures at or i

above the' criteria for structural adequacy, as defined in RG 1.121. The largest cracks were successfully in-situ pressure tested in excess of 3AP. The three largest freespan indications identified this outage were also successfully in-situ pressure tested in excess of 3AP.

ANO-2 utilizes N-16 monitors for primary-to-secondary leakage detection, and has an administrative leakage limit of 0.1 gallon per minute (144 gallons per day), which is less than the ANO-2 TS limit for leakage Abnormal operatirg procedures are in place in the event that leakage i

is detected. Other methods for detecting leakage include a condenser off-gas radiation monitor, steam generator blowdown monitors, and main steam line radiation monitors in addition to the utilization of blowdown grab samples. Entergy Operations is sensitive to the potential rapid progression of tube leakage and will take the necessary measures upon detection, should a primary-to-secondary leak occur. Operators routinely train on primary-to-secondary leaks and tube ruptures utilizing.the simulator.

There was no evidence of primary-to-secondary leakage during the operating interval prior to 2P98 (values were below the minimum detectable level of 5 gallons per day).
