On August 29, 1986, we verbally notified your Mr. T. Westerman of a deficiency involving support installations with concrete attachments spaced in violation of applicable Gibbs & Hill specifications. This is an interim report of a potentially reportable item under the provisions of 10CFR50.55(e).
These installations have been completed and accepted by inspection upon reference to specific " Engineering Evaluation of Spacing Violations" (EESV) forms. These EESV forms were used without supporting design change documents authorizing the specific requirement deviation.
