NOTICE OF VIOLATION
                -Illinois Power Company                                                  Docket No. 50-461 As a ' result of the inspection conducted on October 5-16, November 4-10, and November 16-20~.1987, and in accordance with 10 CFR Part12, Appendix C - General Statement of Policy and Procedure for NRC Enforcement Actions (1986), the following violations were identified:
: 1.          10 CFR 50, Appendix B, Criterion V as implemented by the Illinois Power'(IP)
Nuclear Power Operational Quality Assurance Manual, Chapter 5, " Instructions,
                              - Procedures, and Drawings," requires that activities affecting quality be prescribed by documented instructions and procedures of a type appropriate to the circumstances and that work be accomplished in accordance with the procedures.
Contrary to the above, Procedure 8216.11, " Main Steam Isolation Valve Maintenance" was not properly implemented in two cases:
A. In performing Step 8.9.7 for main steam isolation valve l
No. EIN 1821F028A (MWR C38144), the first action of tightening the bonnet stud nuts was performed at increments of 500 ft/lbs of torque rather than 300 ft/lbs or less as required (461/87035-01A).
B. In performing Step 8.8.20 for main steam isolation valve

;                                    body seat rather than being " gently" lowered as required l-                                  (461/87035-018).
I l                              This is a Severity Level V violation (Supplement I).
l i                2.            10 CFR 50, Appendix B, Criterion XVI as implemented by the Illinois Power Nuclear Power Operational Quality Assurance Manual, Chapter 16, "Correctit Action," requires that conditions adverse to quality
: j.                            be promptly identified and corrected.
1 Contrary to the above, when violation of the procedure for tightening bonnet studnuts was identified, lA above, work continued and no corrective action was taken until all 22 stud nuts were tightened to 500 ft/lbs of torque (461/87035-02).
This is a Severity Level V violation (Supplement 1).
