UPDATE ON DECEMBER 9 SECONDARY SYSTEM PIPE FAILURE AND FATALITIES This information updates PNO-II-86-91.
The Unit 2 feedwater pipe rupture has now caused two fatalities, and four other workers remain hospitalized in serious to critical condition with burns.
Unit I remained at 100 percent power for an additional day after the accident but commenced shutting down last night at 5:30 p.m. (EST) as a precaution to check systems similar to those affected in Unit 2. Unit I was at 400 degrees (F) at 8:00 a.m. this morning and is expected to be in cold shutdown later this afternoon.
Tha NRC site team which was dispatched to the site on December 9 was officially implemented as an Augmented Inspection Team (AIT) on December 10. Additionally, the NRC pipe crack study group composed of persons knowledgeable in metallurgy, water hammer and fracture mecharics, will be briefed on the event during a site visit tomorrow.
Preliminary examination of the pipe appears to indicate extensive erosion / corrosion inside the pipe. Other areas of investigation include condition of feedwater check valves, crosion of other piping, and other conditions that could have contributed to the pipe rupture.
M:dia interest continues.      Both the licensee and the NRC are continuing to respond to num2rous media inquiries, and an NRC information officer remains at the site.
Region 11 is continuing to keep the Commonwealth of Virginia fully informed.
