Probable Consequences of Occurrence The operability of the remaining A.C. sources was verified as per T.S. 3.8.1.1 and no loss of off-site power occurred; therefore, the health and safety of the general public were not affected.
Cause of Event The undervoltage relay (27X) for the IJ diesel output breaker circuit failed in the de-energized position. This caused the breaker to be locked out due to open series contacts in the control circuit for the breaker closing mechanism.
Immediate Corrective Action The remaining A.C. power sources were verified to be operable. The 27X relay was replaced and the diesel output breaker was verified to be operable.
Scheduled Corrective Action No further corrective action is scheduled.
Action Taken To Prevent Recurrence No further action is required.
Generic Implications There are no generic implications associated with this event.
