Failure of Radiation Monitoring System Channel RllA, Containment/Plant Vent Air Particulate Monitor.

CONDITIONS' PRIOR TO OCCURRENCE:

Mode 2 operation, RCS temperature 547°F, RCS pressure 2235 psig, Low Power Physics Testing in progress.

DESCRIPTION OF OCCURRENCE:

During Mode 2 operation, the Containment/Plant Vent Air Particulate Monitoring Channel RllA failed, indicated by an activation of the Filter Failure Alarm. The Shift Supervisor implemented Tech Spec Action Statement #20 for.Limiting Condition for Operation No. 3.3.3.1, Table

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

After removal.of the low speed drive motor inspection revealed metal filings in the a:i;ea of the opposite drive end bell. The rotor was fro*zen in place. The apparent cause of the event was bearing failure.

ANALYSIS OF OCCURRENCE:

The RllA channel was inoperable for a period of less than 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />. Tech Spec 3.4w6.l, Action Statement allows operation to continue for up to 30 days' provided that grab samples of the containment atmosphere are taken at least once per 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />. Since the inoperable channel was returned to service in less than 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />, the grab sample requirement was not required. At the time of the occurrence; Channel RllA was selected to monitor the containment atmosphere. During the time the channel was inoperable, no release was made through the plant vent.

At no_time did the event endanger the general public or site personnel.

CORRECTIVE ACTION:

The low speed filter drive motor was replaced with a spare and a functional test was performed to demonstrate operability. The channel was then returned to service.
