'II Attachment1toAEP:NRC:1193Page2ItshouldbenotedthatthebasesforT/S3/4.6.5.1willneedtobeupdatedtoreflecttheincreasedtimeintervaliftheproposedamendmentisapproved.4,0DISCUSSIONSstemDescritionTheicecondenserisdesignedtoensurethattheoverallsystemwillbeavailabletoprovidesufficientpressuresuppressioncapabilitytolimitthecontainmentpeakpressuretolessthan12psigduringLOCAconditionsassumedintheaccidentanalysis.Inparticular,therequirementsfortheicebedensurethattherequirediceinventorywill1)bedistributedevenlythroughthecontainment.bays,2)containsufficientborontoprecludedilutionofthecontainmentsumpfollowingaLOCA,and3)containsufficientheatremovalcapabilitytocondensethereactorcoolantsystemvolumeduringaLOCA.Currently,theicebedisdeterminedoperablebymeetingvarioussurveillancerequirementswhichinclude1)verifyingicebedtemperatureislessthanorequalto27'Fatleastoncepertwelvehours,2)performingchemicalanalysesoftheicebedtoensureaboronconcentrationof1800ppmandapHof9.0to9.5at25'Catleasteveryninemonths,3)verifyingata95Xconfidencelevelthateachbasketcontainsatleast1220lbsoficeeveryninemonths,4)ensuringadequateflowareaisavailableatleastonceeveryninemonths,5)performingavisualinspectionatleastonceper18monthsoftheinletplenumsupportstructuresandturningvanestoensureicebuildupisnottothepointofindicatingabnormaldegradationoftheicecondenser,and6)verifyingthestructuralintegrityoftheicebasketsbyinspectingarepresentativesampleatleastonceevery40months.Allofthesesurveillancerequirementsaremetbyperformanceofapprovedplantprocedures.Thefirstlineofdefensetodeterminetheoperabilityoftheicecondenserlieswiththeoperator.Asmentionedabove,theicebedtemperatureismonitoredatleastonceeverytwelvehourstoensuretemperaturesarelessthanorequal-to27'F.Thisisaccomplishedinaconservativemannerbyreviewingatleast18differentpointsthroughouttheicecondenserandensuringtheyarealllessthanorequalto27'F.Throughouttheyeartheaverageicebedtemperaturegenerallyrangesbetween16'Fand21'F,wellbelowthe27'Fsetpoint.Inadditiontothesurveillancerequirements,therearealarmsinthecontrolroomwhichwillindicatetotheoperatorifanyofthepointsbeingrecordedreach27'F.Also,weeklyoperatortoursrequiretheoperatorstowalkdowntherefrigerationsystemtoevaluateitsabilitytofunction.Thisincludeswalkingdownthe

~g'Il'~off Attachment1toAEP:NRC:1193Page3chillers,airhandlingunits,andglycolpumpstoensurethattheyareinproperworkingorder.Thetoursalsorequiretheoperatorstoopenasampleoftheintermediatedeckdoorstoensuretheyarenotfrozenshut.Thishelpstoensurethatnoabnormaldegradationoftheicecondenserisoccurringduetocondensationorfrozendrainlinesinlocalizedareas.Thenextlineofdefenseistheperformanceofvariousprocedurestoensuretheicebedisingoodphysicalcondition,consistentwiththeassumptionsusedintheaccidentanalysis.Duetotheninemonthsurveillancerequirement,thesesurveillancesarecurrentlycompletedatleastonceduringafuelcyclewhilethereactorisonlineandonceduringtherefuelingoutage.Thesesurveillancesarereferencedaboveandincludechemicalsampling,icebasketweighing,andflowpassageinspection.ThereasonforthechemicalsamplingistoensuretheboronconcentrationandpHrequirementsaremettoprecludedilutionofthecontainmentsumpfollowingaLOCA.Theminimumweightfigureof1220poundsoficeperbasketcontainsa10Xconservativeallowanceforicelossthroughsublimation.Flowpassageinspectionisperformedtoensuretheabsenceofabnormalicebeddegradationaswouldbeindicatedifaccumulationsexceedthe3/8"specifiedinthetechnicalspecifications.Whenthesesurveillancesareperformedwhileinarefuelingoutage,as-founddataiscollectedpriortoperformingthelaborintensivemaintenanceroutine.Themaintenanceroutinetypicallyincludesweighingasmanybasketsaspossiblewithinanallottedtimeframeandemptyingthebasketsthatwouldnot,basedonoperatingexperience,weighgreaterthan1220poundsat'hestartofthenextrefuelingoutage.Thesubsequentmaintenanceincludesrefillingtheicebasketswithiceandperformingdefrostsoftheicecondenserasnecessarytoensuremaximumheattransfercapabilityduringtheupcomingfuelcycle.Also,significanteffortisdedicatedtocleaningupicethataccumulatesduringtheemptying/refillingprocessoftheicebasketstoensurenoflowblockageoccursduringthesubsequentfuelcycle.Tworemainingsurveillancesareperformedtoensurethatnoabnormaldegradationoftheicecondenseristakingplace.Thefirst,performedonan18monthfrequency;.is-aninspectionoftheinletplenumandturningvanesforfrostoriceaccumulation.Thesecond,performedona40monthfrequencyisavisualstructuralinspectionoftheicebasketstoensurethattheyarefreeofdetrimentalstructuralwear,cracks,corrosionorotherdamage.

~y1L.a Attachment1toAEP:NRC:1193Page4ReasonforProosedTechnicalSecificationChaneReuestTherequesttoincreasethesurveillanceintervalfromninemonthsto18monthswoulddecreasethenumberofevolutionsnecessarytoentercontainmentduringoperationoftheunittoverifyicebasketboronconcentrations,weights,andflowareablockage.Therevisedsurveillanceintervalwouldrequiretheperformanceofsuchicebedmonitoringduringrefuelingoutages.Webelievethatimprovementsinicebedinspectionresultsduetomodifiedmaintenancetechniquesanddesignchangesthathavebeenimplementedsince1984provideadequateassurancethattheicecondensercanmeetandevenexceeditsdesignfunctionwithoutperformingthesurveillancesonaninemonthfrequency.Also,increasingthesurveillanceintervalwouldreducethestressesputonthebasketsandtheirsupportsduetorepeatedweighingevolutions'ustificationforProosedTechnicalSecificationChanesAnincreasedsurveillanceintervalforthechemicalanalysisrequiredbyT/S4.6.5.1.b.lisjustifiedforthefollowingreasons,First,priortorefillingbasketsduringrefuelingoutagemaintenanceactivities,theboratedwatersourceusedtomakeiceissampledtoverifythattheboronconcentrationandpHlevelsaremet.Areviewoflicenseeeventreportsdatingbackthrough1978revealednooccurrencesoffailingtomeetthesurveillancerequirementsofthisspecification.Also,areviewoficecondenserbasketboronconcentrationandpHsampledatabetweenAugust,1986andSeptember,1992forUnit1andbetweenAugust,1986andJanuary,1993forUnit2wasreviewed,andotherthanafewproblemsduetothesamplingtechniques,noproblemswereapparentinmeetingthesurveillancerequirementsofthisspecification.Also,itshouldbenotedthatanincreasetothe18monthfrequencyhasalreadybeenapprovedinthenewstandardtechnicalspecificationsforWestinghouseplantsissuedinSeptember1992bytheNRC.Thefrequencyof18monthswasdevelopedconsideringthesefacts:-1)Longtermicestoragetestshavedeterminedthatthechemicalcompositionofthestorediceisextremelystable,2)OperatingexperiencehasdemonstratedthatmeetingtheboronconcentrationandpHrequirementshasneverbeenaproblem,and3)Someonewouldhavetoenterthecontainmenttotakethesample,and,iftheunitisatpower,thatpersonwouldreceivearadiationdose.

Inparticular, therequirements fortheicebedensurethattherequirediceinventory will1)bedistributed evenlythroughthecontainment.
bays,2)containsufficient borontoprecludedilutionofthecontainment sumpfollowing aLOCA,and3)containsufficient heatremovalcapability tocondensethereactorcoolantsystemvolumeduringaLOCA.Currently, theicebedisdetermined operablebymeetingvarioussurveillance requirements whichinclude1)verifying icebedtemperature islessthanorequalto27'Fatleastoncepertwelvehours,2)performing chemicalanalysesoftheicebedtoensureaboronconcentration of1800ppmandapHof9.0to9.5at25'Catleasteveryninemonths,3)verifying ata95Xconfidence levelthateachbasketcontainsatleast1220lbsoficeeveryninemonths,4)ensuringadequateflowareaisavailable atleastonceeveryninemonths,5)performing avisualinspection atleastonceper18monthsoftheinletplenumsupportstructures andturningvanestoensureicebuildupisnottothepointofindicating abnormaldegradation oftheicecondenser, and6)verifying thestructural integrity oftheicebasketsbyinspecting arepresentative sampleatleastonceevery40months.Allofthesesurveillance requirements aremetbyperformance ofapprovedplantprocedures.
Thefirstlineofdefensetodetermine theoperability oftheicecondenser lieswiththeoperator.
Asmentioned above,theicebedtemperature ismonitored atleastonceeverytwelvehourstoensuretemperatures arelessthanorequal-to27'F.Thisisaccomplished inaconservative mannerbyreviewing atleast18different pointsthroughout theicecondenser andensuringtheyarealllessthanorequalto27'F.Throughout theyeartheaverageicebedtemperature generally rangesbetween16'Fand21'F,wellbelowthe27'Fsetpoint.
Inadditiontothesurveillance requirements, therearealarmsinthecontrolroomwhichwillindicatetotheoperatorifanyofthepointsbeingrecordedreach27'F.Also,weeklyoperatortoursrequiretheoperators towalkdowntherefrigeration systemtoevaluateitsabilitytofunction.
Thisincludeswalkingdownthe  
~g'Il'~off Attachment 1toAEP:NRC:1193 Page3chillers, airhandlingunits,andglycolpumpstoensurethattheyareinproperworkingorder.Thetoursalsorequiretheoperators toopenasampleoftheintermediate deckdoorstoensuretheyarenotfrozenshut.Thishelpstoensurethatnoabnormaldegradation oftheicecondenser isoccurring duetocondensation orfrozendrainlinesinlocalized areas.Thenextlineofdefenseistheperformance ofvariousprocedures toensuretheicebedisingoodphysicalcondition, consistent withtheassumptions usedintheaccidentanalysis.
Duetotheninemonthsurveillance requirement, thesesurveillances arecurrently completed atleastonceduringafuelcyclewhilethereactorisonlineandonceduringtherefueling outage.Thesesurveillances arereferenced aboveandincludechemicalsampling, icebasketweighing, andflowpassageinspection.
Thereasonforthechemicalsamplingistoensuretheboronconcentration andpHrequirements aremettoprecludedilutionofthecontainment sumpfollowing aLOCA.Theminimumweightfigureof1220poundsoficeperbasketcontainsa10Xconservative allowance foricelossthroughsublimation.
Flowpassageinspection isperformed toensuretheabsenceofabnormalicebeddegradation aswouldbeindicated ifaccumulations exceedthe3/8"specified inthetechnical specifications.
Whenthesesurveillances areperformed whileinarefueling outage,as-founddataiscollected priortoperforming thelaborintensive maintenance routine.Themaintenance routinetypically includesweighingasmanybasketsaspossiblewithinanallottedtimeframeandemptyingthebasketsthatwouldnot,basedonoperating experience, weighgreaterthan1220poundsat'hestartofthenextrefueling outage.Thesubsequent maintenance includesrefilling theicebasketswithiceandperforming defrostsoftheicecondenser asnecessary toensuremaximumheattransfercapability duringtheupcomingfuelcycle.Also,significant effortisdedicated tocleaningupicethataccumulates duringtheemptying/refilling processoftheicebasketstoensurenoflowblockageoccursduringthesubsequent fuelcycle.Tworemaining surveillances areperformed toensurethatnoabnormaldegradation oftheicecondenser istakingplace.Thefirst,performed onan18monthfrequency;.
is-aninspection oftheinletplenumandturningvanesforfrostoriceaccumulation.
Thesecond,performed ona40monthfrequency isavisualstructural inspection oftheicebasketstoensurethattheyarefreeofdetrimental structural wear,cracks,corrosion orotherdamage.

Therevisedsurveillance intervalwouldrequiretheperformance ofsuchicebedmonitoring duringrefueling outages.Webelievethatimprovements inicebedinspection resultsduetomodifiedmaintenance techniques anddesignchangesthathavebeenimplemented since1984provideadequateassurance thattheicecondenser canmeetandevenexceeditsdesignfunctionwithoutperforming thesurveillances onaninemonthfrequency.
Also,increasing thesurveillance intervalwouldreducethestressesputonthebasketsandtheirsupportsduetorepeatedweighingevolutions'ustification forProosedTechnical Secification ChanesAnincreased surveillance intervalforthechemicalanalysisrequiredbyT/S4.6.5.1.b.l isjustified forthefollowing reasons,First,priortorefilling basketsduringrefueling outagemaintenance activities, theboratedwatersourceusedtomakeiceissampledtoverifythattheboronconcentration andpHlevelsaremet.Areviewoflicenseeeventreportsdatingbackthrough1978revealednooccurrences offailingtomeetthesurveillance requirements ofthisspecification.
Also,areviewoficecondenser basketboronconcentration andpHsampledatabetweenAugust,1986andSeptember, 1992forUnit1andbetweenAugust,1986andJanuary,1993forUnit2wasreviewed, andotherthanafewproblemsduetothesamplingtechniques, noproblemswereapparentinmeetingthesurveillance requirements ofthisspecification.
Also,itshouldbenotedthatanincreasetothe18monthfrequency hasalreadybeenapprovedinthenewstandardtechnical specifications forWestinghouse plantsissuedinSeptember 1992bytheNRC.Thefrequency of18monthswasdeveloped considering thesefacts:-1)Longtermicestoragetestshavedetermined thatthechemicalcomposition ofthestorediceisextremely stable,2)Operating experience hasdemonstrated thatmeetingtheboronconcentration andpHrequirements hasneverbeenaproblem,and3)Someonewouldhavetoenterthecontainment totakethesample,and,iftheunitisatpower,thatpersonwouldreceivearadiation dose.

Duringthistimeframe,therewerethreeoccurrences inUnit1andfiveoccurrences inUnit2whensomeofthesurveillance requirements werenotmet.T/S4.6.5.l.b.2 requiresthefollowing threecriteriabemettomeeticeinventory requirements.
1)Arepresentative sampleofatleast144icebasketsmustbeweighedanddetermined tocontainatleast1220poundsoficeata95percentlevelofconfidence, 2)Theminimumaverageiceweightsofthesamplebasketsfromradialrows1,2,4,6,8and9ineachofthreegroupsshallnotbelessthan1220poundsoficeata95percentlevelofconfidence, and3)Minimumtotalicecondenser weightata95percentlevelofconfidence shallnotbelessthan2,371,450 pounds.Theoccurrences occurredin1982,1983,and1985inUnit1andin1982,1983,1984,and1986inUnit2.Asaresultoftheproblemsencountered, anicecondenser taskforcewasformedcomposedofplantengineers, AEPSCengineers, andconsulting specialists inrefrigeration.
Thistaskforceprovidednumerousrecommendations toimprovethecoolingcapability oftheicecondenser refrigeration systemanddeveloped bettermaintenance techniques toimproveiceweights.Someoftherecommendations whichhavebeenincorporated asdesignchangesincludeinstalling aprogrammable defrostcontroller forairhandlingunitstoensurethatheatloadsaredistributed evenlythroughout theicecondenser toreduceexcessive sublimation inlocalized areas.Duringrefueling outageicereplenishment activities, toolsandvariousothermaterials hadaccumulated intheductwork,whichcreatedhotspotsintheicebedandincreased sublimation rateswheretheductworkwasblocked.Debrisscreenswereinstalled overwallpanelopeningstoprecludecloggingofductworkusedtocooltheicebed.Also,aflowbalanceoftheductworkwasperformed inbothunitstodistribute coolingmoreevenlytothecranewallinordertoreducesublimation ratesthatwereoccurring inrows8and9.Manyotherdesignchangeshavebeeninstalled toimprovethereliability andperformance oftheicecondenser refrigeration.

Attachment1toAEP:NRC:1193Page6accomplishedbydevelopingtechniquestoemptythebasketcompletelyandthenrefillingitwithnewiceinsteadofaddingwatertoexistingiceorpackingicebyhandtoincreaseiceweight.Thesetechniques,aswellastheabovedesignchangeimplementations,haveledtoanincreaseintheaverageas-foundicebasketweightsofatleast90poundsinUnit1sinceSeptemberof1985andinUnit2sinceDecemberof1984.Itshouldbenotedthatthethirdrequirementfortotalminimumicecondenserweighthasalwaysbeensatisfiedandhasneverbeenanissueofconcern.Thedeficiencieshaveoccurredwhencalculatingaverageindividualandgroupbasketweights.Recommendationsalsoresultedinaheightenedawarenessoftheimportanceoftherefrigerationsystemanditsassociatedmaintenancepracticeswhichhavebeenimprovedtoensurethatairhandlingunitsarerepairedinatimelymanner.Weeklytoursbyoperationspersonnelhavealsobeenincorporatedtoidentifyrefrigerationsystemdeficienciesbeforetheyhaveanadverseeffectontheconditionoftheicecondenser.TheactionsrequiredbyT/S4.6.5.l.a,whichisperformedona12hourinterval,aswellasassociatedalarmsofferadequateindicationforicebedandrefrigerationsystemstatuspriortoanyabnormaldegradationoftheicecondenser.Basedonthepreviousdiscussion,weproposethatthesurveillanceintervalofninemonthsforT/S4.6.5.1.b.2beincreasedto18months.Asimilarrequest,publishedintheFederalRegisteronFebruary20,1991wasmadebyDukePowerCompanyfortheirCatawbaNuclearStation,Units1and2,andwassubsequentlyapprovedbytheNRC.DukePowerCompanyincreasedtheirminimumrequirediceweightsapproximatelyfivepercenttoaccountforexpectedsublimation.WebelievethisisnotnecessaryfortheCookNuclearPlantrequestbecausethepreviousweighingsduringtheUnit1andUnit2refuelingoutagesdatingbackthrough1986havemetallsurveillancerequirementswithoutanyiceadditionsbeingmademid-cycle.TS4.6.5.1.b.3WithrespecttotheflowpassageinspectionsrequiredbyTechnicalSpecification4.6.5.1.b.3,licenseeeventreporthistorieswerereviewedbackthrough1978.Inthattimeframe,thereweresevenoccurrencesbetween1983and1988whichindicatedthattherequirementsofthespecificationhadnotbeenmet.Ineachofthecases,however,itwasconcludedthattheflowpassageswerenotdegradedtosuchadegreethatwouldhaveprohibitedtheice

Basedonthepreviousdiscussion, weproposethatthesurveillance intervalofninemonthsforT/S4.6.5.1.b.2 beincreased to18months.Asimilarrequest,published intheFederalRegisteronFebruary20,1991wasmadebyDukePowerCompanyfortheirCatawbaNuclearStation,Units1and2,andwassubsequently approvedbytheNRC.DukePowerCompanyincreased theirminimumrequirediceweightsapproximately fivepercenttoaccountforexpectedsublimation.
Webelievethisisnotnecessary fortheCookNuclearPlantrequestbecausethepreviousweighings duringtheUnit1andUnit2refueling outagesdatingbackthrough1986havemetallsurveillance requirements withoutanyiceadditions beingmademid-cycle.

Webelievethatthesurveillances performed perT/S4.6.5.1.a ona,twelvehourintervalandtheaddedtoursperformed ontheicecondenser refrigeration systembyoperations personnel providesufficient indication ofabnormaldegradation oftheicecondenser priortoitbecomingaconcern.ItshouldbenotedthatthefailuresthatoccurredperT/S4.6.5.1.b.2 alsooccurredduringasimilartimeframe(1982through1986),andwebelievethedesignchangesimplemented andtheenhancedmaintenance techniques discussed abovehavecontributed toreducingtheamountoficebuild-upintheflowpassages.
Alsotobeconsidered aretheextensive effortsthatareputforthtoensureflowpassagesarecleanedoutfollowing maintenance activities duringarefueling outage.New,methodsweredeveloped in1992toperformlocalized defrosting oftheicecondenser bayswhichhavereducedfrostbuildupontheicecondenser latticeframework alongthecraneandcontainment walls.Basedonthefactthatthesurveillance requirements fortheprocedures havebeenmetsince1988,throughthreemid-cycle inspections, inbothUnit1andUnit2,werequestthattheintervalforinspection beincreased to18months.5'NOSIGNIFICANT HAZARDSDETERMINATION Wehaveevaluated theproposedT/Ssexemption andhavedetermined thatitshouldnotrecpxireasignificant hazardsconsideration basedonthecriteriaestablished in10CFR50.92(c).
Operation oftheCookNuclearPlantinaccordance withtheproposedamendment willnot:(1)Involveasinificantincreaseintherobabilit orconseencesofanaccidentreviouslevaluated.

4,0DISCUSSION SstemDescritionTheicecondenser isdesignedtoensurethattheoverallsystemwillbeavailable toprovidesufficient pressuresuppression capability tolimitthecontainment peakpressuretolessthan12psigduringLOCAconditions assumedintheaccidentanalysis.

Inparticular, therequirements fortheicebedensurethattherequirediceinventory will1)bedistributed evenlythroughthecontainment.

bays,2)containsufficient borontoprecludedilutionofthecontainment sumpfollowing aLOCA,and3)containsufficient heatremovalcapability tocondensethereactorcoolantsystemvolumeduringaLOCA.Currently, theicebedisdetermined operablebymeetingvarioussurveillance requirements whichinclude1)verifying icebedtemperature islessthanorequalto27'Fatleastoncepertwelvehours,2)performing chemicalanalysesoftheicebedtoensureaboronconcentration of1800ppmandapHof9.0to9.5at25'Catleasteveryninemonths,3)verifying ata95Xconfidence levelthateachbasketcontainsatleast1220lbsoficeeveryninemonths,4)ensuringadequateflowareaisavailable atleastonceeveryninemonths,5)performing avisualinspection atleastonceper18monthsoftheinletplenumsupportstructures andturningvanestoensureicebuildupisnottothepointofindicating abnormaldegradation oftheicecondenser, and6)verifying thestructural integrity oftheicebasketsbyinspecting arepresentative sampleatleastonceevery40months.Allofthesesurveillance requirements aremetbyperformance ofapprovedplantprocedures.

Thefirstlineofdefensetodetermine theoperability oftheicecondenser lieswiththeoperator.

Asmentioned above,theicebedtemperature ismonitored atleastonceeverytwelvehourstoensuretemperatures arelessthanorequal-to27'F.Thisisaccomplished inaconservative mannerbyreviewing atleast18different pointsthroughout theicecondenser andensuringtheyarealllessthanorequalto27'F.Throughout theyeartheaverageicebedtemperature generally rangesbetween16'Fand21'F,wellbelowthe27'Fsetpoint.

Inadditiontothesurveillance requirements, therearealarmsinthecontrolroomwhichwillindicatetotheoperatorifanyofthepointsbeingrecordedreach27'F.Also,weeklyoperatortoursrequiretheoperators towalkdowntherefrigeration systemtoevaluateitsabilitytofunction.

Thisincludeswalkingdownthe

~g'Il'~off Attachment 1toAEP:NRC:1193 Page3chillers, airhandlingunits,andglycolpumpstoensurethattheyareinproperworkingorder.Thetoursalsorequiretheoperators toopenasampleoftheintermediate deckdoorstoensuretheyarenotfrozenshut.Thishelpstoensurethatnoabnormaldegradation oftheicecondenser isoccurring duetocondensation orfrozendrainlinesinlocalized areas.Thenextlineofdefenseistheperformance ofvariousprocedures toensuretheicebedisingoodphysicalcondition, consistent withtheassumptions usedintheaccidentanalysis.

Duetotheninemonthsurveillance requirement, thesesurveillances arecurrently completed atleastonceduringafuelcyclewhilethereactorisonlineandonceduringtherefueling outage.Thesesurveillances arereferenced aboveandincludechemicalsampling, icebasketweighing, andflowpassageinspection.

Thereasonforthechemicalsamplingistoensuretheboronconcentration andpHrequirements aremettoprecludedilutionofthecontainment sumpfollowing aLOCA.Theminimumweightfigureof1220poundsoficeperbasketcontainsa10Xconservative allowance foricelossthroughsublimation.

Flowpassageinspection isperformed toensuretheabsenceofabnormalicebeddegradation aswouldbeindicated ifaccumulations exceedthe3/8"specified inthetechnical specifications.

Whenthesesurveillances areperformed whileinarefueling outage,as-founddataiscollected priortoperforming thelaborintensive maintenance routine.Themaintenance routinetypically includesweighingasmanybasketsaspossiblewithinanallottedtimeframeandemptyingthebasketsthatwouldnot,basedonoperating experience, weighgreaterthan1220poundsat'hestartofthenextrefueling outage.Thesubsequent maintenance includesrefilling theicebasketswithiceandperforming defrostsoftheicecondenser asnecessary toensuremaximumheattransfercapability duringtheupcomingfuelcycle.Also,significant effortisdedicated tocleaningupicethataccumulates duringtheemptying/refilling processoftheicebasketstoensurenoflowblockageoccursduringthesubsequent fuelcycle.Tworemaining surveillances areperformed toensurethatnoabnormaldegradation oftheicecondenser istakingplace.Thefirst,performed onan18monthfrequency;.

is-aninspection oftheinletplenumandturningvanesforfrostoriceaccumulation.

Thesecond,performed ona40monthfrequency isavisualstructural inspection oftheicebasketstoensurethattheyarefreeofdetrimental structural wear,cracks,corrosion orotherdamage.

Therevisedsurveillance intervalwouldrequiretheperformance ofsuchicebedmonitoring duringrefueling outages.Webelievethatimprovements inicebedinspection resultsduetomodifiedmaintenance techniques anddesignchangesthathavebeenimplemented since1984provideadequateassurance thattheicecondenser canmeetandevenexceeditsdesignfunctionwithoutperforming thesurveillances onaninemonthfrequency.

Also,increasing thesurveillance intervalwouldreducethestressesputonthebasketsandtheirsupportsduetorepeatedweighingevolutions'ustification forProosedTechnical Secification ChanesAnincreased surveillance intervalforthechemicalanalysisrequiredbyT/S4.6.5.1.b.l isjustified forthefollowing reasons,First,priortorefilling basketsduringrefueling outagemaintenance activities, theboratedwatersourceusedtomakeiceissampledtoverifythattheboronconcentration andpHlevelsaremet.Areviewoflicenseeeventreportsdatingbackthrough1978revealednooccurrences offailingtomeetthesurveillance requirements ofthisspecification.

Also,areviewoficecondenser basketboronconcentration andpHsampledatabetweenAugust,1986andSeptember, 1992forUnit1andbetweenAugust,1986andJanuary,1993forUnit2wasreviewed, andotherthanafewproblemsduetothesamplingtechniques, noproblemswereapparentinmeetingthesurveillance requirements ofthisspecification.

Also,itshouldbenotedthatanincreasetothe18monthfrequency hasalreadybeenapprovedinthenewstandardtechnical specifications forWestinghouse plantsissuedinSeptember 1992bytheNRC.Thefrequency of18monthswasdeveloped considering thesefacts:-1)Longtermicestoragetestshavedetermined thatthechemicalcomposition ofthestorediceisextremely stable,2)Operating experience hasdemonstrated thatmeetingtheboronconcentration andpHrequirements hasneverbeenaproblem,and3)Someonewouldhavetoenterthecontainment totakethesample,and,iftheunitisatpower,thatpersonwouldreceivearadiation dose.

Duringthistimeframe,therewerethreeoccurrences inUnit1andfiveoccurrences inUnit2whensomeofthesurveillance requirements werenotmet.T/S4.6.5.l.b.2 requiresthefollowing threecriteriabemettomeeticeinventory requirements.

1)Arepresentative sampleofatleast144icebasketsmustbeweighedanddetermined tocontainatleast1220poundsoficeata95percentlevelofconfidence, 2)Theminimumaverageiceweightsofthesamplebasketsfromradialrows1,2,4,6,8and9ineachofthreegroupsshallnotbelessthan1220poundsoficeata95percentlevelofconfidence, and3)Minimumtotalicecondenser weightata95percentlevelofconfidence shallnotbelessthan2,371,450 pounds.Theoccurrences occurredin1982,1983,and1985inUnit1andin1982,1983,1984,and1986inUnit2.Asaresultoftheproblemsencountered, anicecondenser taskforcewasformedcomposedofplantengineers, AEPSCengineers, andconsulting specialists inrefrigeration.

Thistaskforceprovidednumerousrecommendations toimprovethecoolingcapability oftheicecondenser refrigeration systemanddeveloped bettermaintenance techniques toimproveiceweights.Someoftherecommendations whichhavebeenincorporated asdesignchangesincludeinstalling aprogrammable defrostcontroller forairhandlingunitstoensurethatheatloadsaredistributed evenlythroughout theicecondenser toreduceexcessive sublimation inlocalized areas.Duringrefueling outageicereplenishment activities, toolsandvariousothermaterials hadaccumulated intheductwork,whichcreatedhotspotsintheicebedandincreased sublimation rateswheretheductworkwasblocked.Debrisscreenswereinstalled overwallpanelopeningstoprecludecloggingofductworkusedtocooltheicebed.Also,aflowbalanceoftheductworkwasperformed inbothunitstodistribute coolingmoreevenlytothecranewallinordertoreducesublimation ratesthatwereoccurring inrows8and9.Manyotherdesignchangeshavebeeninstalled toimprovethereliability andperformance oftheicecondenser refrigeration.

system.Maintenance techniques developed includedusingvibrators toemptybasketsanddeveloping toolstoreplacecruciforms inthebaskets.Thistaskforceisstillintactandconvenesperiodically toreviewthestatusoficecondenser issues.Asrecommended bythetaskforce,maintenance techniques werealsoenhancedtoincreasetheiceweightsinbasketstoensureminimumtechnical specification requirements wouldbemetwithouthavingtoshutdownthereactormid-cycle toreplenish iceinventory.

Attachment 1toAEP:NRC:1193 Page6accomplished bydeveloping techniques toemptythebasketcompletely andthenrefilling itwithnewiceinsteadofaddingwatertoexistingiceorpackingicebyhandtoincreaseiceweight.Thesetechniques, aswellastheabovedesignchangeimplementations, haveledtoanincreaseintheaverageas-foundicebasketweightsofatleast90poundsinUnit1sinceSeptember of1985andinUnit2sinceDecemberof1984.Itshouldbenotedthatthethirdrequirement fortotalminimumicecondenser weighthasalwaysbeensatisfied andhasneverbeenanissueofconcern.Thedeficiencies haveoccurredwhencalculating averageindividual andgroupbasketweights.Recommendations alsoresultedinaheightened awareness oftheimportance oftherefrigeration systemanditsassociated maintenance practices whichhavebeenimprovedtoensurethatairhandlingunitsarerepairedinatimelymanner.Weeklytoursbyoperations personnel havealsobeenincorporated toidentifyrefrigeration systemdeficiencies beforetheyhaveanadverseeffectonthecondition oftheicecondenser.

TheactionsrequiredbyT/S4.6.5.l.a, whichisperformed ona12hourinterval, aswellasassociated alarmsofferadequateindication foricebedandrefrigeration systemstatuspriortoanyabnormaldegradation oftheicecondenser.

Basedonthepreviousdiscussion, weproposethatthesurveillance intervalofninemonthsforT/S4.6.5.1.b.2 beincreased to18months.Asimilarrequest,published intheFederalRegisteronFebruary20,1991wasmadebyDukePowerCompanyfortheirCatawbaNuclearStation,Units1and2,andwassubsequently approvedbytheNRC.DukePowerCompanyincreased theirminimumrequirediceweightsapproximately fivepercenttoaccountforexpectedsublimation.

Webelievethisisnotnecessary fortheCookNuclearPlantrequestbecausethepreviousweighings duringtheUnit1andUnit2refueling outagesdatingbackthrough1986havemetallsurveillance requirements withoutanyiceadditions beingmademid-cycle.

TS4.6.5.1.b.3 Withrespecttotheflowpassageinspections requiredbyTechnical Specification 4.6.5.1.b.3, licenseeeventreporthistories werereviewedbackthrough1978.Inthattimeframe,thereweresevenoccurrences between1983and1988whichindicated thattherequirements ofthespecification hadnotbeenmet.Ineachofthecases,however,itwasconcluded thattheflowpassageswerenotdegradedtosuchadegreethatwouldhaveprohibited theice

Since1988therehasbeennoindication ofabnormalicebeddegradation withrespecttomeetingtherecpxirements ofT/S4.6.5.1.b.3.

Webelievethatthesurveillances performed perT/S4.6.5.1.a ona,twelvehourintervalandtheaddedtoursperformed ontheicecondenser refrigeration systembyoperations personnel providesufficient indication ofabnormaldegradation oftheicecondenser priortoitbecomingaconcern.ItshouldbenotedthatthefailuresthatoccurredperT/S4.6.5.1.b.2 alsooccurredduringasimilartimeframe(1982through1986),andwebelievethedesignchangesimplemented andtheenhancedmaintenance techniques discussed abovehavecontributed toreducingtheamountoficebuild-upintheflowpassages.

Alsotobeconsidered aretheextensive effortsthatareputforthtoensureflowpassagesarecleanedoutfollowing maintenance activities duringarefueling outage.New,methodsweredeveloped in1992toperformlocalized defrosting oftheicecondenser bayswhichhavereducedfrostbuildupontheicecondenser latticeframework alongthecraneandcontainment walls.Basedonthefactthatthesurveillance requirements fortheprocedures havebeenmetsince1988,throughthreemid-cycle inspections, inbothUnit1andUnit2,werequestthattheintervalforinspection beincreased to18months.5'NOSIGNIFICANT HAZARDSDETERMINATION Wehaveevaluated theproposedT/Ssexemption andhavedetermined thatitshouldnotrecpxireasignificant hazardsconsideration basedonthecriteriaestablished in10CFR50.92(c).

Operation oftheCookNuclearPlantinaccordance withtheproposedamendment willnot:(1)Involveasinificantincreaseintherobabilit orconseencesofanaccidentreviouslevaluated.

Theincreaseinthesurveillance intervaldoesnotcreateasignificant increaseintheprobability orconsequences ofanaccidentpreviously evaluated.

Recentoperating experience withtheicecondenser indicates thatallrequiredboronconcentrations, pHlevels,iceweights,andfrostoriceaccumulation criteriaestablished intheT/Sshavebeenmetwithoutanycorrective orpreventive actionbeingtakenduringthemid-cycle inspections.

Thisprovidesconfidence thattheicecondenser willcontinuetobeabletoperformasassumedinthesafetyanalysis.

Therefore, weconcludethattheproposedT/Sschangesdonotinvolveasignificant increaseintheprobability orconsequences ofanaccidentpreviously evaluated.

Theabovereviewfoundnopossibility ofanewordifferent kindofaccidentfromanypreviously analyzed.

(3)Involveasinificantreduction inamarinofsafetTheproposedT/Sschangesonlychangethesurveillance frequency requirements whichwebelievewillnotchallenge theabilityoftheicecondenser toperformitsfunctionasdefinedinthesafetyanalysis.

OtherT/Ssandplantindications areinplacetowarntheoperatorofrefrigeration systemproblemspriortopossibleicebeddegradation.

Therefore, weconcludethattheT/Schangedoesnotinvolveasignificant reduction inamarginofsafety.6.0PENDINGTSsPROPOSALS IMPACTING THISSUBMITTAL Therearecurrently nootherT/Ssproposals underreviewthatwouldimpactthissubmittal,
