| The caps were replaced and all other hatches, Unit I and II, were verified secured.

| The following actions were assigned to prevent reoCCurrenCO.
| |
| 1.
| |
| The local leak rate test procedures are to be revised to require specific signoff steps assuring that the test connections are isolated.
| |
| 2.
| |
| Plant preheatup containment integrity checklists are to be revised to include local leak rate connections on hatches.
| |
| 3.
| |
| Design changes will be initiated as necessary to install isolation valves for local leak rate test connections.
