On November 9,1998, a representative of Intemational Radiography and Inspection l       Services, Inc. notified the NRC Operations Center of an incident that occurred at a                     I l       temporary jcbsite located in Sapulpa, Oklahoma. A preliminary dose assessment for the radiographer's assistant involved in the incident has been completed by the licensee's Radiation Safety Officer. The preliminary calculations determined that the assistant may have received a whole body dose of approximately 18 rems and an extremity dose of approximately 150 rems to the right hand. The licensee intends to pursue further blood analysis to attempt to more accurately assess the dose received by the assistant during the event. The State of Oklahoma and NMSS have been informed.
