The Technical Specification requirement to take grab samples every eight hours has been observed during the entire period of RER Service Water Radiation Monitor inoperability. | At the start of the outage, the "A" and "C" monitors were taken out-of-service to accomodate the modification. | ||

The magnetic Flow Switch Assembly, used to initiate the " Low Sample Flow" alarm, was inoperable. | |||

All work in the Hot Maintenance Shop was discontinued and the ventilation was immediately shutdown. The ventilation had been in service for approximately 18 hours without sample flow through the radiation monitor prior to discovery. There were no adverse consequences as a result of the event. No work was being performed in the shop which involved components with high contamination levels during the period in which the radiation monitor was not functioning. Other air samplers were in service in the area and did not indicate any airborne contamination concern. INTRODUCTION f
This submittal complies with the format described in Regulatory Guide 1.21, " Measuring, Evaluating and Reporting Radioactivity in Solid Wastes and Releases of Radioactive Materials in Liquid and Gaseous Effluents from Light-Water Cooled Nuclear Power Plants", Revision 1, June, 1974 with data summarized on a quarterly basis following the format of Appendix B thereof.
In accordance with LGS Technical Specification 6.9.1.8, this document summarizes the gaseous effluent releases for the period of January 1, 1987 through June 30, 1987.
The plant was able to achieve its internal goal of NO liquid releases during this report period.
Where "0.00E+00" is used, it denotes the less than detectable level for the given isotope.
Supplemental information, consistent with the format of Appendix B of Regulatory Guide 1.21, is also included.
Disposition of solid waste during the report period (January 1, 1987 to June 30, 1987) is described within to include: the total activity shipped by waste type and an estimate of the error in the reported totals; the' estimated composition of each type of waste by isotope; and the solid waste disposition, i.e. number of shipments, mode of transportation, destination, type of container, total container volume, and solidification agent.
An explanation of why inoperable Residual Heat Removal Service Water Radiation Monitors (A,B, and C) were not returned to service within the time specified is provided in accordance with Technical Specification 3.3.7.11.
An explanation of why the Hot Maintenance Shop ventilation was in-service with less than the minimum number of radioactive gaseous effluent monitoring instrumentation channels operable and without compensatory sampling taking place as required is provided in accordance with Technical Specification 3.3.7.12.
There were no changes to the Offsite Dose Calculation Manual (ODCM) or Process Control Program (PCP) during the report period.
There were no major changes to Radioactive Waste Treatment Systems during the report period.

RESIDT" L HEAT REMOVAL (RHR) SERVICE WATER RADIATION MONI" A FAILURES During the First Refueling Outage, a modification to replace all four (4) RHR Service Water Radiation Monitors was initiated.
At the start of the outage, the "A" and "C" monitors were taken out-of-service to accomodate the modification.
i At this time, the "B" monitor was inoperable due to failure of the Low Sample Flow Switch during performance of a Surveillance Test.
The magnetic Flow Switch Assembly, used to initiate the " Low Sample Flow" alarm, was inoperable.
A new flow switch was ordered, but upon delivery was determined to be incorrect.
Delays incurred to reorder and install the correct switch resulted in the thirty (30) day action statement being exceeded.

radiation monitors are out-of-service and the "C" monitor is in service The Technical Specification requirement to take grab samples every eight hours has been observed during the entire period of RER Service Water Radiation Monitor inoperability.

Hot Maintenance Shop Radiation Monitor Failure i
During the First Refueling Outage, the Hot Maintenance Shop ventilation was discovered to be operating without the associated exhaust radiation monitor functioning and without compensatory sampling taking place as required by Technical Specification 3.3.7.12.
All work in the Hot Maintenance Shop was discontinued and the ventilation was immediately shutdown.
The ventilation had been in service for approximately 18 hours2.083333e-4 days <br />0.005 hours <br />2.97619e-5 weeks <br />6.849e-6 months <br /> without sample flow through the radiation monitor prior to discovery.
There were no adverse consequences as a result of the event.
No work was being performed in the shop which involved components with high contamination levels during the period in which the radiation monitor was not functioning.
Other air samplers were in service in the area and did not indicate any airborne contamination concern.
This discrepancy is discussed in further detail in Licensee Event Report No.87-031.
