The cause of the large number of doors being inoperable at one time is that all of the doors are being inspected once every six months during approximately a one-week period.

This leads to a 3arge number of doors needing to be repaired following each inspection.

As a result, it takes too long to accomplish the repairs.

In addition, too low a priority has been assigned to the repair of fire doors.

The high failure rate is a consequence of the lengthy period between inspections for certain of the doors.

Corrective actions will consist of:

Because of the large number of fire doors that were found to be inoperable in this inspection, we intend to switch to a monthly, staggered inspection in place of the six-month inspection schedule now used.

Additional quarterly maintenance will be conducted on doors which exhibit a high failure rate.

Finally, a higher priority will be assigned to the repair of inoperable safety related fire doors in the future, with a goal of one week to accomplish any needed repairs.

The compensatory measures of Trojan Technical Specification 3.7.9 (checking smoke detectors and establishing an hourly fire patrol) were established as required.
