                                          NOTICE OF VIOLATION

          Arizona Nuclear Power Project                              Docket No. 50-528

          As a result of the inspection conducted during the period August 1, througn
;        September 8, 1985, and in accordance with NRC Enforcement Policy, 10 CFR Part
          2, Appendix C, the following violations were identified.
          1.    Technical Specification 3.7.12 requires that each unlocked fire door
                without electrical supervision located in fire rated assembly
                penetrations shall be verified closed at least once per 24 hours.

i              During the period April 17, through August 11,'1985, three unlocked fire
                doors without electrical supervision (J-lli, A-336 and J-126) located in
                fire rated assembly penetrations, were not verified to be closed, and a
                fourth door (A-203) was verified closed on a'seven day cycle rather than
                once per 24 hours, as required.
                This is a repeat feverity Level IV Violation (Supplement 1).

!        2.    Technical Specification 6.2.2.2 states, in part, that (a) Administrative
i'              procedures shall be developed and implemented to limit the working hours
                of unit staff who perform safety-related functions, and that. (b)
                Operating personnel who perform safety-related work in excess of 16 hours
                in any 24 hour period shall have their overtime approved by the PVNGS
                Manager or his designee.
                Contrary to the above requirements, (1) administrative procedures to
                limit working hours of the unit staf f support engineers who perform
;              safety-related functions had not been developed and implemented, and (2)
'
                a member of the unit staff (Operations Engineering Department) worked 20
'
                hours during the 24 hour period beginning at 6:00 PM on August 19, 1985
                without the overtime being approved by the PVNGS Plant Manager or his
                designee.' During the overtime period the engineer.took measurements and
                calculated percent of valve travel during the performance of safety
                related surveillance testing to satisfy Technical Specification
                requirement 4.5.2.g, which involves verifying the correct position of
                electrical and mechanical stops associated with specific emergency core
                cooling throttle valves.
                This is a severity level IV Violation (Supplement 1).

          Pursuant to the provisions of 10 CFR 2.201, Arizor.a Public Service Company is
          hereby required to submit to this office within thirty days of the date of
          this Notice, a written statement or explanation in reply, including: (1) the
          corrective steps which have been taken and the results achieved; (2)
          corrective steps which will be taken to avoid further items of noncompliance;

          and (3)'the date when full compliance will be achieved.
          Consideration may be given to extending your response time for good cause
          shown.
