The customer identified an abnormal curve trace (resistive short) on the integrated circuit installed at location IC825 pin 6 on this printed circuit board. The customer performed an evaluation which included dissection of the subject IC [integrated circuit] however, this dissection provided inconclusive results as to the cause of the IC anomaly."At this time, UCI requires additional time to evaluate the results provided by PSEG and to work with the manufacturer in identifying the root cause. UCI currently considers this issue to be an isolated event as no previous instances of this failure have been observed by UCI to date. 1OCFR21 Interim Letter, UCI Not Capable to Complete Part 21 Evaluation Within Timeline The purpose of this letter is to provide the NRC with a report in general conformity of the requirements of 10CFR Part 21.21. On January 30, 2015 United Controls International issued a Return Material Authorization (RMA) request to Public Service Electric & Gas (PSEG). The RMA was issued to request the return of a Sense & Transfer Module of part number 41-09-339904 that was provided to PSEG under purchase order 4500726553.
10CFR21 Interim Letter, UCI Not Capable to Complete Part 21 Evaluation Within Timeline The purpose of this letter is to provide the NRC with a report in general conformity of the requirements of 10CFR Part 21.21. On January 30, 2015 United Controls International issued a Return Material Authorization (RMA) request to Public Service Electric & Gas (PSEG). The RMA was issued to request the return of a Sense & Transfer Module of part number 41 339904 that was provided to PSEG under purchase order 4500726553. The customer identified an abnormal curve trace (resistive short) on the integrated circuit installed at location IC825 pin 6 on this printed circuit board. The customer performed an evaluation which included dissection of the subject IC, however, this dissection provided inconclusive results as to the cause of the IC anomaly.

UCI shall determine whether the component installed at board location IC825 is indeed the root cause of failure. The manufacturer has been requested to provide the soonest available date for testing and is expected to be performed within the next two months.(viii) Any Advice related to the defect or failure to comply about the facility, activity, or basic component that has been, Is being, or will be given to purchasers or licensees.
