DOCKET NOS. 50-445 AND 50-446 SUPPORT INSTALLATIONS - SPACING VIOLATIONS SDAR: CP-86-59 (FINAL REPORT)

Gentlemen:

On August 29, 1986, we verbally Notified your Mr. T. Westerman of a deficiency involving support installations with concrete attachments in violation of applicable CPSES design specifications.

Our latest interim report logged TXX-6109 was issued November 24, 1986. This issue is applicable to Units 1 and 2 and affects pipe, conduit, cable tray, and other support installations.

We have concluded the deficiencies identified above are reportable under the provisions of 10CFR50.55(e) and the required information follows.

DESCRIPTION Engineering Evaluation Separation Violation (EESV) forms are used to document support spacing design problems which require engineering resolution.

Modification to a support design resulting from the engineering resolution requires the initiation of the appropriate design change documents to implement the change.

Our evaluation has identified the following deviations from the established design change control program.

1.

EESV forms have been issued for support installations without the required design char;ge documents. These installations were accepted by Quality Control (QC) based on technical data provided by the EESV form.

2.

Engineering disciplines have, in some cases, not controlled the issue / distribution of EESVs such that documented interdiscipline evaluation of all areas of the design and installation has rot been assured.

3.

Procedural control of the EESV criteria has been inadequate.

.whereby the function of the basic components (supports) can not-be assured during seismic events.

CORRECTIVE ACTIONS Procedure ECE-3.06-15 " Evaluation-and Documentation of Concrete Attachment Spacing Violations" has been established to effectively. control, transmit and document spacing violations. This procedure-is scheduled for issue by June 15, 1987, and specifically assigns and clarifies engineering and inspection responsibilities. To validate the support and structure interface and to assure the integrity of the installations, an interdiscipline review will be performed.

In addition, QC will perform a 100% reinspection of the installations using revised design and design ~ change documents.

The status of any modification required as a. result of this deficiency will be reported as corrective actions for the deficiency documented per SDAR: CP-86-04.
