SHIPMENT | On November 30,1998, Sperry-Gun Drilling Services, Inc. (Sperry) shipped an Amersham Model CDC.CY13 logging source containing 2 curies of Cs-137 (Serial No. 2299GW) via Federal Express (FedEx). The package was picked up in Houston, Texas, and was to be shipped to the north slope of Alaska. On December 8,1998, the licensee contacted FedEx to determine the location of the package as it had not been received by Sperry's facility at the north slope. On December 15,1998, FedEx contacted the licensee and reported that the package could not be found. FedEx indicated that they were able to track the missing source through FedEx's transportation hub in Indianapolis, Indiana. However, no information was obtained regarding further shipment of the source from Indianapolis, Indiana, to Anchorage, Alaska. FedEx has determined that the source is not at its Indianapolis hub. | ||
