REQUEST FOR COMPUTER PRINTOUT ADDRESSEE LABELS The State of Ohio became an Agreement State on August 31,1999. In addition to NRC licensees in Ohio, there are a number of other NRC licensees that need to be informed of the agreement. This is to request computer printout addressee labels for licensees nctionwide within the following program codes in order to make all proper notifications.

03121 03225 03251 03620 22170 For program codes 02220,02240, and 02500, only address labels for licensees in the States adjoining Ohio, i.e., Indiana, Kentucky, Pennsylvania, and West Virginia are requested.

We would also like to have addressee labels for general licensees in Ohio who are registered with NRC pursuant to 10 CFR 31.11 (" Registration Certificate - In Vitro Testing With Byproduct Material Under General License," Form NRC-483). These licensees will be under Ohio authority after the Agreement becomes effective.
