Report Submitted By: Wes Stilwell, Westinghouse Electric Company, LLC Report Date: February 23, 2021 Report ADAMS Accession No.: ML21054A141 Review the incoming report to determine if additional Commission or staff action is warranted. The review should consider whether the report identifies a generic defect or problem with the package design and the safety significance of the issue.

Note that a high safety significance represents a potential for significant radiation exposure, medium safety significance represents a potential for some moderate radiation exposure, and low safety significance represents little or no potential for radiation exposure.

1. The report identifies:

__ Significant reduction in the effectiveness of a package during use;

__ Defect with a safety significance; Shipment in which conditions of the approval were not observed.

3. Summary of the report:

In the application for Certificate of Compliance No. 9291, Revision No. 11, for the Liqui-Rad package, the certificate holder requested an increase of the lower limit for the torque values on the outer lid bolts. The U.S. Nuclear Regulatory Commission issued Revision No. 11 on February 13, 2019 (ADAMS Accession No. ML18317A352), which changed the lower limit from 30 [+10 / -10] ft-lbs to 30 [+10 / -0] ft-lbs, as specified in the operating procedures provided with the application.

In response to the revised certificate Westinghouse changed its internal procedure for operating the package. However, the internal Westinghouse procedure that was issued on July 25, 2019, listed the torque for the outer lid nuts as 30 [+10 / -10] ft-lb, which did not comply with the operating procedure referenced in the certificate. When Westinghouse noticed the discrepancy on December 18, 2020, it placed a hold on all shipments for this package, until the procedure was revised. Westinghouse stated that the incorrect lower limit on the torque value was incorrectly recorded in the procedure due to human error.

Westinghouse stated that approximately 150 shipments, each containing nine packages, were made from their plant in Columbia SC between July 25, 2019 and December 18, 2020. However, Westinghouse confirmed that there were no known shipments that were out of compliance with the revised certificate of compliance. In addition, Westinghouse confirmed there were no known issues resulting from the lower outer lid lower torque value in their procedures.

4. Corrective actions taken by the licensee:

Westinghouse included this event in its corrective action program and completed the following actions:

  • Issued a revised operating procedure to correct the torque values,
  • Reviewed the Liqui-Rad offloading procedures to ensure no other non-conforming issues or conditions existed, and
  • Provided coaching to its staff that perform procedure updates.
5. Staff comments:

The NRC reviewed the report submitted by Westinghouse and determined this incident is neither safety significant nor indicative of a trend.

6. Staff conclusion:

The report does NOT identify generic design or license/certificate issues that warrant additional Commission or staff action. This report is considered closed.
