                                                                      .PY-CEI/NRR-0750 L' Page'1 of 1 SPECIAL REPORT DIVISION 2 DIESEL' GENERATOR VALID FAILURE On' October 15,:1987 a valid failure occurred on the Division 2 Diesel Generator while performing Technical Specificctien Surveillance 4.8.1.1.2. This is the sixth valid failure to occur on'the Perry Unit 1, Division 2 Diesel Generator.
As of October 15, 1987 the Division 2 Diesel Generator had undergone 61' valid tests,' including this sixth valid failure.
The failure occurred'on the~ start sequence of the Division 2 Diesel Generator j        during a routine surveillance test. .. A shuttle valve apparently failed to-l        reposition'on the start signal'which prevented the permissive signal to the field flashing circuit. Vithout the permissive.the generator did;not flash.and therefore produced no output, voltage. .The Division.2 Diesel Generator was manually shutdown without incident. The' shuttle valve vas replaced and a design change initiated to replace;its function with electro-mechanical relays.
        .The Division 2 Diesel Generator was restarted and verified operable by performing the Division 2 Diesel Generator and Load Surveillance Test. ' The Division 2 Diesel Generator was declared. inoperable on October 15, 1987 at 0910 and was subsequently declared operable on October 17,.1987 at 1312 for a total out of service time of 2 days, 4 hours and 2 minutes. Since that time the design change has been implemented and successfully retested.
The surveillance testing interval vill remain at,once every 7 days, which is in accordance with Technical Specification Table 4.8.1.1.2-1.
