ThefollowingspecificitemsmentionedintheNoticeofViolationareexamplesofthefirstreason.ArecentreviewofthemicrofilmrecordcopyindicatedthattheentireprofilesfortheRockbestoscable(Unit2EQFile6.3)andfortheITTBartonTransmitters(Unit2EQFile9;2)wereintherecordcopyatthetimeoftheinspectionandhadbeenxeroxedincorrectlywhenthehardcopiesweremade.(Theprofileswereonfoldoutpagesandwerecopiedinthefoldedposition.)SimilarlyforRomecable(Unit1EQFile19.0),theentireFranklinReportisintherecordcopyeventhoughpagesmayhavebeenmissinginthehardcopythatwasinspected.(ItisunclearatthistimewhetherthepagesweremissingfromthehardcopysincearecentreviewofthehardcopyindicatedthatallthepagesoftheFranklinReportarethereandthelatestrevisiondateis4/25/85.)Thefollowingitemsareexamplesofthesecondreason.IntheTECAcousticFlowMonitoringSystemfiles(Unit1EQFiles22.0and23.0andUnit2EQFile8.5),themildenvironmentandseismicportionsofthereporthadbeenremovedtoconservepaperonUnit1butwereincludedonUnit2(thefileswereassembledbydifferentconsultants.)Sincemildenvironmentandseismicqualificationareoutsidethescopeof10CFR50.49thiswasconsideredacceptable.Also,therewasnoreasontodoubtthattheappropriatetestinghadbeendoneandthattheactualdatatakenduringthetest(andconsideredproprietarybyTEC)wasavailableforreviewatthevendor'sfacilitiesforthefollowingreasons.1)ThetestreportsreceivedfromTECindicatedthatthe,testingprogramperformedbyTECwasverycomplete.2)AproceduredescribingthefunctionaltestingtobeperformedduringtheLOCAwasreferencedinthefiles.3)AworddescriptionofthefunctionalperformanceofthesystemduringtheLOCAwascontainedinthefiles.4)FPL'sQAdepartmenthadauditedandapprovedTEC'sQAprogram.Thereforeaspecialaudittoreviewthedatawasdeemedunnecessary.SimilarcircumstancescanbenotedfortheGarrettSolenoidValves(Unit2EQFile35.9)andtheVictoreenRadiationMonitoringSystem(Unit1EQFile26.0).TheGarrettSCEWsheetdidnotrequireaoneyearpostDBAoperability(Unit2hasamaximumpost'BAof180days)andduringtheinspectionwasdemonstratednottoberequiredatall.Forty(40)yearlifewasdemonstratedbytheagingdoneinthetestreportandtheArrheniuscalculationdoneinthetestreportprocedurethatwereinthefile.Applicabilityofthereportstothevalves'installedwasdemonstratedbytwoCombustionEngineeringlettersandtheuseofgasketsthatwerenotpre-irradiatedwasdispositionedbyGarrettthroughsupplementaltestingandsimilarityanalysis.TheVictoreenqualificationPage2of4 wasbasedonanalysisbackedupbyactualconditionsexperiencedatTNIwhichexceededtherequirementsatUnit1.Theseagainwereinthefilesat-thetimeoftheinspectionandconsideredadequatebyFPL.Thefinalitem,GeneralCable(Unit1EQFile8.0),istheresultofthethirdreason.Thequestionofairthermalagingqualificationaroseonthemorningofthelastdayoftheinspectionwithnotimetorespond.FPLhaddevelopedseveralmethodsintheEQFiletodemonstratequalificationoftheGeneralXLPEcable,thestrongestofwhichwasconsideredtobetheRaychemreportqualifyingtheirXLPEcablewhichwaspre-aged.Asecondmethodalso"consideredacceptableusedtheGeneralCable'LOCAreportontheirXLPEcablethatwasnotpre-agedandtheoriginal79-01BaginganalysisontheGeneral'able.SincethisitemwasidentifiedtoFPLjustpriortotheinspectionexitmeeting,therewasn'ttimetopointthisouttotheinspector.3.TheEQFilesreviewedbytheinspectorshavebeenconvertedfromrecordfilestoEnvironmentalQualificationDocumentationPackages(docpacs)whicharelivingdocumentspresentlycontrolledundertheFPLdrawingprogram.Commentsmadebytheinspectorshavebeenincorporatedintothedocpacs.t4.AllEQFileshavebeenreviewed,revisedandconvertedintodocpacsandplacedunderthecontrolofthedrawingprogram,5.WorkbeganimmediatelyaftertheApril1986inspectiontoconverttheEQfilestodocpacs.ThefirstreviseddocpacswereissuedinJune1986with60percentoftheworkcompletedbytheendofSeptember1986.ThelastoftheoriginalfileswasconvertedandissuedbyJanuary30,1987.However,sincethefilesarenowlivingdocuments,theywillbecontinuouslyrevisedandupdated.Page3of4 FINDINGB10CFR50.49(f)statesinpart,thateachitemofelectricalequipmentimportanttosafetyshallbequalifiedbytestingoforexperiencewith,identicalorsimilarequipment,andqualificationshallincludesupportinganalysistoshowthattheequipmenttobequalifiedisacceptable.Contrarytotheabove,atthetimeoftheinspection,environmentalqualification(EQ)FileNo.39.1(Unit2)didnotadequatelydemonstratequalificationofRDFResistanceTemperatureDetectors(RTD),inthatdatarequiredtosupportaqualifiedlifeoftheRTVsealantusedinthedetectorswerenotincludedinthefile.RESPONSE1.FloridaPower&LightCompany(FPL)concurswiththefinding.2.DuringtheCombustionEngineering(CE)LOCAtestingoftheRTD'saproblemwithmoistureintrusiondeveloped.Tocorrectthisahightemperature,radiationresistantRTVwasusedtopottheRTDheadandLOCAtestingwascompleted.Twoletters(CEletterL-CE-8878dated6/29/83andEbascoletterP-M-SL-84-035dated1/10/84includedinthefileprior'othetimeoftheinspection)documentedthatalthoughtheRTVwasnotagedorirradiateditwouldbequalifiedfora22monthperiod(actualArrheniuscalculatedlifebasedondatafromtheEbascoletterwas81.44'years)andworkbegantoredesigntheRTDheadorfindasuitablereplacementfortheRTD's.ItwasdecidedtoreplacetheRTD's,however,suitablequalifiedreplacementscouldnotbefoundorobtainedintimetosupporttheoutage.ThereforethegasketsandRTVwerereplacedduringtheoutageto,keeptheCE/Ebascolettersvalidforanadditionalrefuelingcycle.Duringtheinspection,theinspectordeterminedthatthiswasanunacceptablemethodtodemonstrateagingqualification.3t4,DuringtheinspectionanadditionalcalculationwasdonewhichdemonstratedthattheLOCAtesthadsufficientmargintoqualifytheRTVforovera40yearlifeaswellastherequiredDBA.Theinspectoragreedthatthiswasacceptableandthatthecalculationshouldbeincorporatedintothefile.Thiswasdoneduringthereviewandconversionofthefilesintodocpacs.AllEQFileshavebeenreviewed,revisedandconvertedintodocpacsandplacedunderthecontrolofthedrawingprogram.05.Inadditiontoincorporatingthecommentsmadebytheinspector,informationfromGeneralElectricontheRTVwasobtainedandincludedinthenewRDFRTDdocpac.ThedocpacwasthenissuedinJanuary1987.Page4of4
