1 NOTICE OF VIOLATION Commonwealth Edison Company                          Docket Nos. 50-454; 50-455 As a result of the inspection conducted from October 31 through November 30, 1987, and in accordance with the " General. Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the following violations were identified:                                                                      !
: 1. Technical Specification 4.6.3.1 requires that main steam isolation valve bypass valve 2MS101C shall be demonstrated operable, prior to its return to service after maintenance, by the performance of a cycling test and by verification of its isolation time.
Contrary to t'ie above, on September 3, 1987, valve.2MS101C was returned to service following maintenance without the performance of an isolation time test.                                                                                  i i
This is a Severity Level IV violation (Supplement I). (455/87038-02(DRP))
: 2. 10 CFR 50, Appendix B, Criterion II, as implemented by Commonwealth                          j Edison Company's Quality Assurance Manual, Quality Requirement (QR) 2.0,                    i requires that the licensee's quality assurance program shall provide for                    1 indoctrination and training of personnel performing activities affecting quality as necessary to assure that suitable proficiency is achieved and maintained.
1 Quality Requirement 2.0, Section 2.3, requires that personnel engaged in                    I testing activities will be assigned according to their formal education and on-the-job training. Additionally, training programs will be developed to indoctrinate and qualify personnel in specific testing activities in which they will be engaged.
Quality Procedure (QP) 2-52 implements the requirements of QR 2.0 and states that it applies to the administration of training programs for                    -
operations, maintenance, technical, management, and quality assurance functions in the production area so that personnel achieve and maintain suitable proficiency in the performance of their assigned tasks and responsibilities. QP 2-52 requires that the production training manager prepare training standards for applicable production area personnel.
A training standard is a document which identifies the training requirements for a given position.
Figure A of the Byron Station Addenda to the Commonwealth Edison Company's Quality Assurance Manual indicates that technical staff engineers are part of the station production department.
The performance of Technical Specification testing on emergency core cooling system pumps, which is required by the American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code, Section XI, kb 0

is an activity affecting quality. The discharge flowrate of each pump is one of several parameters which are measured to verify that the pump's performance has not degraded.                                                        j Contrary to the above:
: a. Cn November 19, 1987, a technical staff engineer performed an ASME surveillance test on the 2A centrifugal charging pump, without having received any training on the use of the ultrasonic flowmeter which he used to determine the discharge flowrate of the pump,
: b. By November 19, 1987, training standards had not been developed for technical activities (operation of the ultrasonic /lowmeters) that are performed by the Byron technical staff engineers.
This is a Severity level IV violation (Supplement I). (454/87041-01(DRP);
455/87038-03(DRP)).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1)correctiveaction taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
