Report of Personnel and Man-Rom by Work and Job Function This report is a tabulation on an annual basis of the number of station, utility, and other personnel (including contractors) who received exposures greater than 100 mrem / year and their associated man-rem exposure according to work and job function.
The work and job function totals for personnel greater than 100 mrem / year are different than the grand totals because an individual may perform work under more than one work l        andjob function during the year.

!        The total man-rem is based on a ratio of the official TLD results.
;        A description of the Special Plant Maintenance performed in 1998 is as follows:
I e  Radiation Work Permit 19981023 was for installation of modification WX-033 per Engineering Change Notices (ECNs) 29711, 29712, 29713, on the Unit 1 Fuel Pool Cooling and Clean-up Filter Demin Sludge Tank. Room decontamination was also permitted under this Radiation Work Permit (RWP).
* Radiation Work Permit 19981053 was for building / removing scaffold and shielding, tagging out and draining the Residual Heat Removal (RHR) heat exchanger (HX) "A",
HX room decontamination, removing / installing channel cover, cleaning the Shutdown Service Water (SX) side of RHR HX tubes, performing eddy current testing, and torquing the channel cover.
l e  Radiation Work Permit 19981081 was fbr a pre-job walkdown to verify feasibility of design installation of modification RTF039.
l e  Radiation Work Permit 19981084 was for tagout, installation / removal of shielding, erect / wreck scaffold, cutout / move / weld a new valve in a new location in the Back Wash Receiving Tank Room mezzanine. Work activities included grinding, surface preparation, decontaminate / clean pipe internals, welding and weld inspections.
* Radiation Work Permit 19981112 was for work performed per Maintenance Requests (M.Rs) D77852 and D77853 to remove existing thermo-lag, hanger, conduit and electrical work, and to install a new fire barrier on cable trays.
* Radiation Work Permit 19981123 was for installing scaffold and rigging beam for Emergency Core Cooling System (ECCS) Suction Strainer modification activities.
Modification work included cutting hand rails, grinding paint from embeds, installing temporary lighting, beam hangers, fire blanket for welding activities, and running welding leads.
* Rrdiation Work Permits 19981124 and 19981125 were for vacuuming the Suppression Pool.

i e Radiation Work Permit 19981226 was for replacing and rerouting line ISF0.28A-2" and associated work which included: erect / wreck scaffold, cutting / welding, painting, disposal of old line, area decontamination and Radiation Protection (RP) technician coverage, e Radiation Work Permit 19981127 was for diving work in the Suppression Pool for ECCS Suction Strainer Replacement Modification.
e Radiation Work Permit 19981136 was for Auxiliary Building Steam Tunnel (ABST) work to support modification FW-039. Work included: scaffolding, piping / hanger layout and installation, coatings, borescope inspections, Quality Assurance (QA)/ Inservice Inspections (ISI), post maintenance testing (PMT), and RP technician support.
              . Radiation Work Permit 19981137 was for performing radiography on FW-039 modification piping wekts in the ABST.
* Radiation Work Permit 19981138 was for scafTolding and shielding installation and removal in support of FW-039 modification activities in the "A" and "B" RHR HX rooms. Support activities include grating removal and RP Technician and ALARA personnel support.
* Radiation Work Permit 19981139 was for modification FW-039 work activities.
Work included installing hangers, piping and valves; drilling; Bisco seal removal / replacement; core drill; electrical work; painting; insulation removal / replacement; post-heat treatment; QA/ISI; PMT; and RP Technician and ALARA personnel support.
* Radiation Work Permit 19981140 was for performing radiography in the RHR "A" HX room for the FW-039 modification.
e Radiation Work Permit 19981141 was for performing radiography in the gas control boundary in support of the FW-039 modification.

Radiation Work Permit 19981142 was for performing FW.039 modification work                    i activities to include: pipe / hanger layout; installing hangers and pipe; installing conduit and shielding; tie-in RHR pipe (drilling); insulation removal / replacement; painting;        l PMT; QA; RP fiberscope inspection; temoval of debris and shielding; electrical and penetration work; and Bisco repair.

        'e      Radiation Work Permit 19981143 was for performing radiography in support of the FW-039 modification.
* Radiation Work Permit 19981169 was for installation of hot taps on SX piping for IVY 01S, IVYO2S, and IVYO4S per MR D86265 in the Low Pressure Core Spray (LPCS), RHR "A" and Reactor Core Isolation Cooling (RCIC) pump rooms. Work included non-destructive examination (NDE), painting, insulation removal / installation, scaffold support, grinding, drilling, welding, hydrolazing, ultrasonic testing (UT), and flow test.
* Radiation Work Permit 19981170 was for installation of hot tap on SX piping for IVYO9S in Main Steam Isolation Valve (MSIV) Blower Room. Work included NDE, painting, insulation and scaffold installation / removal, drilling, UT, grinding and welding, hydrolazing, and flow testing.
* Radiation Work Permit 19981171 was for installing hot tap on SX piping for 1E12-C002A in the RHR "A" Pump room. Work included NDE, painting, insulation and scaffold installation / removal, drilling, UT, grinding and welding, hydrulazing, and flow testing.
* Radiation Work Permit 19981172 was for installing hot tap on SX piping for IVY 03S in RHR "A" HX Room. Work included NDE, painting, insulation and scaffold installation / removal, drilling, UT, grinding and welding, hydrolazing, and flow testing.

e      Radiation Work Permit 19981181 was for installing a fire barrier wall on the 707 foot elevation of the Auxiliary Building general walkway. Included all support activities.        I o      Radiation Work Permit 19981183 was for performing radiography in Fuel Building                  j room F1-5 in support of the FW-039 modification.
        .      Radiation Work Permit 19981194 was for installing a lifting device frame per instructions in ECN 30842. This work included scaffold erection / wrecking.                    .
* Radiation Work Permit 19981195 was for installing hot taps on SX piping for IVY 10S in ti.e MSIV Blower Room. Work included painting, removing / installing shielding, insulation and scaffold support, grinding, welding, hydrolazing, boroscoping, UT, grid pip and flow testing.
        .      Radiation Work Permit 19981196 was for performing radiography in RHR "B" HX                      )
room, 720 foot elevation of the Auxiliary Building, and the MSIV Blower Rooms per preventive maintenance tasks PMMSXM006 and PMMSXM007.

* Radiation Work Permit 19981272 was for erecting / wrecking scaffolding, shielding / insulation removal and installation in suppon ofinstalling a bypass line on valves 1E12-F014B and IE12-F068B. Installation of bypass line required drilling and welding on SX piping.
