                                                          Region IV personnel in Arlington, Texas to discuss the licensee's position on

                                                          the seven apparent violations, one deviation, one unresolved item, and several
                                                          concerns addressed in NRC Inspection Report 50-072/88-01; 50-407/88-01.

                                                          The attendance list is attached as Appendix A to this summary.    The meeting was                          -l
                                                          held at the request of the Region IV Regional Administrator.                                                  l

                                                          The licensee representatives responded to all violations, the deviation, the                                  ;
                                                          unresolved item, and all of the open items identified in the NRC Inspection                                    ;
                                                          Report roted above. The licensee took exception to several of the inspection                                  l
                                                          findings and provided the following comments:

                                                          1.    Apparent Violation (407/8801-02): The licensee stated that the cause of

                                                                the unexpected criticality was not the reinstallation of the improperly                                f-

                                                                reassembled heavy water tank, but the lack of knowledge on the part of the                              i
                                                                reactor operator concerning reactivity changes in the core due to fuel                                  L

                                                          2.    Apparent Violation (072/8801-06): The licensee stated that they had
                                                                discussed the removal of the bricks with the NRC Project Manager, prior to
                                                                their removal.
                                                          3.    Apparent Violation (072/8801-07; 407/8801-08): The licensee stated that

                                                                they had sent the revised PSP to the NRC within the proper time frame, but
                                                                that as of the date of the inspection they had not received notification                                i

                                                                from the NRC concerning their submittal.                                                                '

                                                          4.    Unresolved Item (072/8801-08; 407/8801-08): This item was resolved by the                                i
                                                                licensee's verification that the University Police Department responded                                  l

                        The licensee's discussions on several of the violations gave the impression
                        that the two licensed senior reactor operators at the facility were not
                        provided sufficient oversight and supervision, and that the reactor
                        supervisor was not sufficiently familiar with NRC regulations and all of the
                        on going activities associated with the incorporation of a computerized system
                        for TRIGA reactor parameter monitoring.
                        The licensee stated they would be submitting a letter to Region IV formally
                        addressing each of the violations, the deviation, and other concerns (open
                        items) identified in the NRC inspection report.
                        The NRC acknowledged the licensee's presentation and stated that the Region IV
                        recommendation concerning enforcement action for the apparent violations would
                        be forwarded to the NRC Of fice of Enforcement for their concurrence. After
                        review by that office, the licensee would be notified in writing of the NRC's
                        proposed enforcement action.
                        Subsequent to the enforcement conference the NRC Region IV contacted the
                        licensee on March 29, 1988, and requested that further operation of the
                        University of Utah reactors be curtailed and that the reactors be shut down
                        pending the NRC review of their response to the Notice of Violations.
                        Confirmatory Action Letter 88-06 was issued to the licensee on March 30, 1988,
                        verifying this understanding.
