This revised form requires that all information be completed in total with each new, renewal, upgrade, multi-unit, and reapplication license submittal. Applicants requesting a renewal and reapplication will also need to indicate the additional training received since the last application under Item 15. Please note that the training section will no longer be cumulative. Other changes are detailed in Enclosure 2.
