13 Containment Fan Coil Unit - Inoperable CONCITIONS PRIOR TO OCCURRENCE:
Mode 1 - Rx Power 99% - Unit Load 1100 MWe DESCRIPTION OF OCCURRENCE:
At 1120 hours on May 12, 1981, Action Statement 3.6.2.3 was entered when the attempt to shift No. 13 Containment Fan Coil Unit from low cooling water flow to high flow was deemed unsuccessful when the required cooling water flow rate was not obtained.              Specifically, the required flow rate of > 2500 gpm was not reached; the flow rate of N 900 gpm was sustained.
This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The designation of the apparent cause of this occurrence is equipment failure. The preliminary investigation of the problem revealed that service water valve 13SW65 did not automatically shift position to provide the higher cooling water flow required. In order to identify the specific problem, the subject valve was dismantled and inspected.
The results of this inspection identified the specific cause of the problem to be an equipment failure when the valve stem was found to be separated from the actuator due to a cracked *runnion block plug.
ANALYSIS OF OCCURRENCE:
Restore the inoperable group of cooling fans to cperable status within 7 days or be in at least hot standby within the next 6 hours and in cold shutdown within the following 30. hours IAW Technical Specification 3.6.2.3.
 
    ,  LER 61-46/03L                                                June 8,              1981 CORRECTIVE ACTION:
The trunnion block plug for 13SW65 was replaced. In addition, and as a preventive maintenance measure, the following valve parts were also replaced while the subject valve was dismantled: snap ring, rod end bearing, feedback spring assembly, disphragm, and the vane arm pin.
The subject valve was subsequently reassembled, tested, and returned to service. Action Statement 3.6.2.3 was terminated at 2335 hours on May 13, 198.'.
