SUBJECT: NRC INSPECTION REPORT 030-21031/99-02 FORM 591

Dear Mr. Stangl:
This letter and the enclosed NRC Form 591 refer to the safety inspection completed on
September 21,1999, of activities authorized under NRC Byproduct Materials
License 50-23404-01. Based on the results of this inspection, the NRC has determined that
one violation of NRC requiraments has occurred. This violation is cited in the enclosed NRC
Form 591.
Pursuant to the provisions of 10 CFR 2.201, you are required, within 30 days of the above date,
to sign and date the enclosed NRC Form 591, indicating that appropriate action will be talcen to
correct the violation described on the form. After printing your name, signing the
NRC Form 591, please keep one copy and retum the remaining copy to me at the address
above.
Please be advised that the results of our deliberations regarding issues discussed at the
pre-decisional enforcement conference held in Anchorage, Alaska, on September 27,1999, will

be addressed in separate correspondence.
