Cn September 26, 1975 during a rcutine inspecticn cf Hydraulic Shcek Suppresscrs (snubbers) inside the secondary shield cf the Reactor Building, one snubber located at pcsition RC-13 cn the Pressurizer Auxiliary Spray Line was fcund to be disecnnected.

Descriptien of Occurrence:

Cn September 26, 1975 during a rcutine inspecticn cf Hydraulic Shcek Suppresscrs (snubbers) inside the secondary shield cf the Reactor Building, one snubber located at pcsition RC-13 cn the Pressurizer Auxiliary Spray Line was fcund to be disecnnected.

The Unit was i==ediately removed, rebuilt, tested and re-installed at locaticn RC-13 (7)

Designation of Apparent Cause of Occurrence :

The cause of this occurrence has been determined to be precedure/

persennel in that on April 7,1975, the snubber installed at RC-13 was found incperable due to a lov fluid level. Due to material and scheduling problems, an operable unit was =cved frcm a locatien cut-side the Reacter Building and installed at RC-13.

Additionally, en May 28, 1975, a report en snubber inspection inside the secondary shield indicated that the RC-13 snubber was removed to be repaired.

Upon re-installation of the snubber at positien RC-13, it apparently was not properly cennected.

Analysis of Occurrence:

It has been determined that this occurrence did not ccnstitute a threat to the health and safety of the public in that:

a.

The probability of occurrence of an e rthquake in the area is lev, and b.

the conservatism of the piping design is such that one snubber may be incperable with a necligible loss of seismic protection since the

. other 13 snubbers en this line vould provide adequate prctection.

Corrective Action:

In additien to the i==ediate action described abcVe,1cng term corrective acticns are as follows:

a.

After ec=pletien of all snubber verk and prior to Reacter Startup, an inspection of all snubbers inside cf the secendary shield was conducted to insure that all snubbers had been preperly re-connected.

b.

The documentatien a:2d centrol associated with the snubber pregra vill be reviewed and revised as necessary.
