Marble Hill Nuclear Generating Station - Units 1 and 2

Dear Mr. Fiorelli:

In accordance with the provisions of Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, Public Service Company of Indiana, Inc. (PSI) of fers the following information in response to the item of noncompliance identified in Inspection Report 50-546/80-28 and 50-547/80-28.

Items of Noncompliance (50-546/80-28-01; 50-547/80-28-01) 10 CFR 50, Appendix B, Criterion XII states in part that, " Measures shall be established to assure that tools, gauges, instruments, and other measuring and testing devices used in activities af fecting quality are properly controlled, calibrated, and adjusted at specified periods to maintain accuracy within necessary limits."

PSAR Marble Hill Quality Assurance Program - Chapter 17 (PSI letter Shields to Denton dated April 28, 1980) Section 1.12 states in part that, " PSI quality assurance specification requires that contracting organizations performing quality-related activities requiring use of measuring and test equipment establish measures to assure that tools, gauges, instruments, and other measuring and testing devices are of the proper type, range and accuracy for their application. To assure accuracy, the measuring devices are properly cont rolled, calibrated, and adjusted at specified periods or prior to use as required by approved procedures."

PSI Project Management Manual Procedure PMP 6.03, Revis, ion 0, Section 5.3, dated March 8,1980, requires the use of a generic matrix (Attachment 4) for maintenance of mechanical equipment and materials. Contrary to the-above:

PSI and their contractors failed to include the inert gas pressure gauges on some equipment in storage and in place in the calibration program. Examples of these are as follows:

Unit 1 Steam Generators and Pressurizer Unit 1.and 2 Regenerative Heat Exchangers Unit 1 and 2 Excess Letdown Heat Exd..ngers Unit 1 and 2 Letdown Heat Exchangers Unit 1 and 2 Seal Water Heat Exchangers Unit 1 and 2 Residual Heat Exchangers Unit 1 and Unit 2 Component Cooling Water Heat Exchangers Corrective Action Taken and Results Achieved Corrective Action Request Number PC-0490 was initiated on July 9,1980 to document -that, contrary to the requirements of ANSI N45.2, Section 13, measures to ensure that " tools, gauges, instruments and other inspection, measuring and testing equipment and devices used in activities affecting quality are of proper range, type and accuracy to verify conformance to established requirements" had not been instituted, in that no program for calibration of purge pressure gauges on items requiring ' inert gas blankets -

existed.

Storage and maintenance instructions have been revised to require calibrated gauges and have been issued to contractors.

Calibrated gauges are currentiv being installed on all purge items requiring inert gas blankets.

Corrective Actions to Prevent Recurrence A system shall be implemented to assure positive pressure within specified tolerances is maintained on required equipment in storage, either through the use of installed calibrated gauges or verification of positive pressure by the use of portable calibrated gauges.
