Dear Mr. Knuth:

Notice is hereby given, as required by 10 CFR 20.405, of the. ' exposure of six individuals to concentrations of radinactivo material in excess of the maximum permissible concentrations allowed by our Technical Specification, Section 6.4.2. In addition, related to the same activity, there was an apparent violation to Technical Specification Table 6.4-1, footnote 2bi which involved three additional individuals. J f% bXE ^ p,y' '1 O~On 18 April 1975, while plugging tubes in the "A" Steam Generator, g, - six individuals entered the tent around the steam generator manway using masks for which the protection factor was 100, into an atmosphere which was 770 times mpc (Appendix B, Table I, Column 1 of 10 CFR 20). All involved individuals were given whole body counts after their exposure to determine the uptake of radioisotopes and only one showed an uptake above maximum permissible, limits.. The others ranged between 1 and 20% of per-missible burden. All except one had received training in the use of masks e.,.rf.. and had used them properly prior to this incident. All signed the Special h' J L.p Work Permit issued for use during blasting, indicating they understood the M requirements for the job which included use of a supplied air _ mask. For these the results of whole body counts show less PIh; than 40 MPC hours of exposure to radioactive materials. Therefore no dose estimates are included in this report. WN k~d Whole body counts will be continued on the one individual who 8 5j showed above permissible body burdens on his initial count. As of this .ih$ date, he has been counted six times since the exposure. The 4/25/75 I%h count (7 days after exposure) represents the material which is slowly JQy removed from the lung and an amount of iodine which would result in a 2N total thyroid dose of 610 mrem. The dose to the lung from Cobalt and 7., Zirconium is 9.6 Rem, for the first year, based on the standard biological &ca half-life of 120 days. This value may be revised on the basis of additional g$ @ ~,j body counts, .. Careful radiation dose records were maintained on all who worked in this area. Corrective Actions: 1. Check sheet for mask use will be incorporated. 2. Escorts and supervisors will be advised of the necessity for closer supervision of contract personnel when they are working in high radiation, and airborne activity areas or on jobs in which radiological conditions are changing. EN The Director of Inspection and Enforcement of the Nuclear Regula-tory Commission has proposed fining the Rochester Gas & Electric Corporation $10,000 for alleged noncompliance with NRC regulations governing radiation protection of workers at RG&E's R. E. Ginna Nuclear Power Flant in Wayne County, New York, relating to an incident that occurred during April 1975. The incident allegedly resulted in a worker being exposed to. con.- centrations of airborne radioactivity, in restricted areas where he and others were working, in an amount that exceeded permissible limits con-tained in NRC regulations. The notice of violation sent by NRC to the company also alleged it failed to use prescribed respiratory protective equipment of a proper type and failed to assure proper supervision and training of personnel who were to use this equipment. In a letter to the company, Dr. Donald F. Knuth, Director of Inspection and Enforcement, asked the company to inform NRC of the steps which have been or will be taken to " strengthen management control" of the radiation safety program at the Ginna plant. The company also was informed that it has 20 days from the receipt of Dr. Knuth's letter to either pay the proposed civil penalty or to protest it in writing, stating its reasons why all or part of the pro-posed fine should not be imposed.
