On August 24, 1989. TU Electric notified the NRC via Reference 3 of deficiencies involving pipe support construction which were determined to be reportable under the provisions of 10CFR50,55(e). Due to the similarities between these deficiencies and those identified in Notices of Violation (NOVs) 445/8912-V-03 and 445/8906-V-02 (Unit 1 only), and SDAR-CP 86-063 (Units 1 and 2), TU Electric is expanding the scope of SDAR-CP-89-011 to encompass the HOVs.and SDAR-CP-86-063. Therefore, all future correspondence on these issues will be addressed as part of SDAR-CP-89-011.
BACKGROUND Reference 1 identified a broken cotter pin on a Unit 1 large bore pipe support (SDAR-CP-86-063). At the time Reference l.was issued, the scope of this SDAR l                                          was limited to Unit I and common safety-related pipe supports.                                                Reference 2 l                                          described additional deficiencies (missing, broken, or loose parts: misaligned components) and described the corrective actions for Unit 1 (Hardware Validation Program).          In subsequent reports, the scope was expanded to include Unit 2. The corrective actions described included performance of inspections as a part of support certification. A more comprehensive discussion of these actions for Unit 2 is provided in " CORRECTIVE ACTION" below.                                                                                I

TXX 91403 Page 2 of 3 Additional pipe support deficiencies (missing, broken, or loose parts as well as misaligned components) were identified by Notices of Violation 445/8912 V-03 and 445/8906 V 02. These deficiencies were reported via Reference 3 (SDAR-CP-89-011). This submittal detailed Unit I corrective actions and stated that Unit 2 corrective actions would be assessed upon resumption of Unit 2 engineering activities. A discussion of corrective actions follows.
CORRECTIVE ACTION Appropriate specifications and construction / inspection procedures have been reviewed to ensure required installation and/or inspection criteria are specified. Unit 2 safety-related supports (ASME safety class 1, 2 and 3 supports and Non-ASHE class 5 supports within an ASME stress problem boundary) installed and inspected prior to issuance of revised criteria have been placed in an "in process" status.      These will be reinspected by Quality Control (OC) to a revised attribute list which addresses the original group 1. 2 and 3 observations / findings identified in Reference 3 and the type of deficiencies addressed in Reference 2. Unit 2 supports installed or inspected subsequent to revision-of the criteria will be encompassed by the normal Construction Engineering (CE)/0C verifications and OC final inspections. This will ensure that the safety-related supports in Unit 2 are inspected to a common set of attributes.
OC personnel will also perform walkdowns as a part of the program for NRC Bulletin 79-14 and ASHE N-5 certification. Walkdowns will also be performed prior to turnover of systems to Startup and subsequently to Operations by Construction Engineering and Startup respectively. Controlled punchlists will identify incomplete work, missing components, or damaged equipment.        Access control to the areas containing pipe supports will be established at a reasonable: interval 1 based on completion status and maintained until final turnover-to Operations. : Periodic surveillance will be conducted to help assure that pipe supports are properly installed and maintained throughout the construction and startup phases.
An assessment of applicable CPSES procedures was completed for rework control.
configuration, maintenance, modifications, and commodity completeness-as these subjects apply to TV Electric Construction. Quality Control. Start-up and Operations. As a result of this assessment, the following activities have been or will be performed:
Inspection attribute lists and applicable procedures'have been revised to incorporate verification of support installation completeness. Startup procedures will be revised to allow the return of pipe. support work on items in Startup custody to Construction to be worked in accordance with the Construction OC program.

TXX 91403 Page 3 of 3 Construction has implemented an increased awareness program which includes:
A general policy memorandum which cautions against damaging installed hardware.
Temporary posting of signs highlighting this policy in various rooms / areas.
The Unit 2 Project Training Program includes an indoctrination program which provides instruction to individuals as to responsibilities to properly perform work so as not to damage installed components and to promptly report any damaged or defective plant components.
The Nuclear Overview Department (NOD) will develop and implement a procedure prescribing its involvement in the room / area turnover program and in monitoring commodities.                        N0D will monitor procedural compliance of the organizations responsible for the inspection and walkdown programs.
The above corrective actions will be completed prior to Unit 2 fuel load.
