The New York Power Authority (NYPA) submits this special report in accordance with Technical Specification 3.12.F.2 and 6.9.B.2.            The report describes fire barrier penetration seals which were not restored to functional status within a seven day period due to planned outage activities.
 
== Introduction:==
 
During the conduct of the Spring, 1994, Maintenance Outage, floor plugs in the Recirculation Motor Generator Set Room were removed to provide maintenance access to the Main Steam Line Tunnel located directly below this room.            Although the Steam Tunnel is'also accessible from the Turbine Building, floor plug access is preferred for ALARA considerations.
The floor plugs functionally provide fire barrier protection between the Recirculation Motor Generator Set Room and the Steam Tunnel.
Action Taken:
On April 4, 1994, a roving fire watch was posted in the Recirculation Motor Generator Set Room and floor. plugs removed to provide maintenance access to the Main Steam Tunnel.          On April 15, 1994,'the fire watch was upgraded to continuous when the installed smoke detection system was taken out of service for scheduled outage maintenance. The smoke detection system was restored and a roving fire watch pse          on April 22, 1994.

Fire watches, either roving or continuous, remained posted until the floor plugs were replaced on April 28, 1994. The plant was shutdown and in the cold condition with the mode switch in Refuel during the time the floor plugs were
.              removed.
Cause:
The scope of work in the steam tunnel was such that it could not be completed within a seven day period. Because the steam plugs were removed for greater than seven days, the Technical Specification requires a special report.
